彩色多普勒超声对胎盘早剥的诊断及临床治疗的指导意义

摘　　要：目的探讨彩色多普勒超声在胎盘早剥(尤其是不典型胎盘早剥)中的诊断价值及临床指导意义。方法选取2014年1月-2017年12月51例胎盘早剥的患者,其中包括23例不典型胎盘早剥患者,对其进行超声图像分析,并将其结果与病理结果进行对照,从而进行诊断符合率的统计。结果51例患者经彩色超声多普勒检查后,确诊为胎盘早剥的46例,与病理结果对照,诊断符合率为90.2%,其中包括19例不典型胎盘早剥患者,误诊2例,误诊率为3.9%,漏诊5例,漏诊率为9.8%,其中不典型胎盘早剥患者4例。在51例胎盘早剥患者中,40例附着于胎盘前壁,5例附着于后壁,6例附着于侧壁。结论彩色多普勒超声可以有效的诊断胎盘早剥,并能确定胎盘早剥的位置,但对于临床表现隐匿的不典型胎盘早剥患者,漏诊率较高,仍需要结合其他临床检测手段。Objective To explore the diagnostic value and clinical guiding significance of color Doppler ultrasound in placental abruption(especially atypical placental abruption).Methods From January 2014 to December 2017,51 patients with placental abruption diagnosed according to clinical manifestations,including 23 patients with atypical placental abruption,were analyzed by ultrasound,The results were correlated with pathology,and the diagnostic coincidence rate was obtained.Results In 51 patients,46 patients with placental abruption were diagnosed by color Doppler ultrasound.The diagnostic coincidence rate was 90.2%,including 19 patients with atypical placental abruption,2 cases were misdiagnosed,the misdiagnosis rate was 3.9%,5 cases were missed,and the missed diagnosis rate was 9.8%,among which 4 cases were atypical placental abruption.Of the 51 patients with placental abruption,40 were attached to the anterior wall of the placenta,5 to the posterior wall,and 6 to the side wall.Conclusions Color Doppler ultrasonography can effectively diagnose placental abruption and determine the location of placental abruption.However,for patients with atypical placental abruption with clinical manifestations,the rate of missed diagnosis is high,and other clinical detection methods are still needed.
