摘 要:目的 探讨骨桥蛋白(OPN)和转化生长因子-β1(TGF-β1)在免疫球蛋白A(IgA)肾病中的表达及其与Haas病理分型的相关性。方法 随机抽取2016年1月至2019年1月长治医学院附属和济医院收治的IgA肾病患者50例,其中Haas分型Ⅰ型6例,Ⅱ型9例,Ⅲ型15例,Ⅳ型12例,Ⅴ型8例。另抽取同期体检健康者25例。比较不同受试者肾组织OPN、TGF-β1、血尿氮素(BUN)、血肌酐(SCr)、尿微量白蛋白(mALB)水平,并分析其与病理的相关性。结果 受试者OPN、TGF-β1水平随型级逐级上升(P<0.05),受试者mALB、BUN、SCr水平随型级逐级上升(P<0.05)。IgA肾病病理分型与OPN、TGF-β1水平呈正相关(r=0.820、0.764,P<0.05),OPN、TGF-β1水平与mALB、BUN、SCr水平呈正相关(P<0.05)。结论 TGF-β1、OPN表达水平与其Haas病理分型、肾功能受损程度呈正相关,二者可能参与了IgA肾病肾间质受损过程。Objective To investigate the expression of osteopontin(OPN)and transforming growth factor-β1(TGF-β1)in immunoglobulin A(IgA)nephropathy and its correlation with the Haas pathological classification.Methods A total of 50 patients with IgA nephropathy admitted to the Heji Hospital Affiliated to Changzhi Medical College from January 2016 to January 2019 were randomly selected,including 6 cases of Haas type I,9 cases of Haas type II,15 cases of Haas type III,and 12 cases of Haas type IV,8 cases of Haas type V.In addition,25 patients with healthy physical examinations were enrolled.The levels of OPN,TGF-β1,blood urea nitrogen(BUN),serum creatinine(SCr)and urinary microalbumin(mALB)in kidney tissues of different subjects were compared and their correlation with pathology was analyzed.Results The levels of OPN and TGF-β1 in the subjects increased gradually with the increase in grade(P<0.05).The levels of mALB,BUN and SCr in the subjects increased gradually with the increase in grade(P<0.05).The pathological type of IgA nephropathy was positively correlated with OPN and TGF-β1 levels(r=0.820,0.764,P<0.05).The levels of OPN and TGF-β1 were positively correlated with the levels of mAb,BUN and SCr(P<0.05).Conclusions OPN and TGF-β1 are involved in the process of renal tubulointerstitial injury in IgA nephropathy patients,and their expression levels are positively correlated with the Haas pathological classification and the degree of renal impairment.
