摘　　要：目的探讨白细胞介素-8(IL-8)、C-X-C趋化因子受体4(CXCR4)在多发性骨髓瘤(MM)患者骨髓活检组织中表达及临床意义。方法收集63例初诊为多发性骨髓瘤患者骨髓组织样本及30例检测为正常骨髓象的骨髓组织样本,进行IL-8、CXCR4免疫组织化学实验(BOND MAX法)检测,分析其在骨髓活检组织中的表达情况,并探讨IL-8、CXCR4表达与临床病理特征的关系。结果MM患者IL-8、CXCR4阳性表达率明显高于对照组,差异有统计学意义(P<0.01),骨髓瘤R-ISS分期增高,IL-8、CXCR4阳性表达率增高;多发性骨髓瘤的骨髓组织IL-8表达与CXCR4表达呈正相关性(r=0.458,P<0.01);患者年龄、IL-8表达、CXCR4表达均可作为独立判断MM患者预后因素(P<0.05)。结论IL-8、CXCR4的表达量检测对MM的R-ISS分期、预后均有一定的判断价值。Objective To investigate the expression and clinical significance of IL-8 and CXCR4 in bone marrow biopsy of patients with multiple myeloma(MM).Methods A total of 63 cases of bone marrow tissue samples from patients with multiple myeloma and 30 samples of normal bone marrow tissue were collected and detected for the immunohistochemistry(BOND MAX)of IL-8 and CXCR4.The expression of bone marrow biopsy tissue was analyzed,and the relationship between the expressions of IL-8 and CXCR4 and clinicopathological characteristics was explored.Results The positive expression rates of IL-8 and CXCR4 in MM patients were significantly higher than those in the control group,with the differences statistically significant(P<0.01).The R-ISS stage of myeloma increased,and the positive rates of IL-8 and CXCR4 increased.The expression of IL-8 in bone marrow tissue of multiple myeloma was positively correlated with the expression of CXCR4(r=0.458,P<0.01).The age,IL-8 expression and CXCR4 expression can all be used as independent prognostic factors in MM patients(P<0.05).Conclusion The detection of IL-8 and CXCR4 levels has certain value in predicting the R-ISS stage and prognosis of MM.
