摘　　要：目的探讨胸腰椎骨折患者使用经伤椎置钉法进行短节段椎弓根钉内固定术的效果。方法选取县级医院收治的胸腰椎骨折患者各23例进行临床对照试验,其中A组予以经伤椎置钉短节段椎弓根钉内固定术(SSPS-IFTV)治疗,B组予以跨伤椎置钉短节段椎弓根钉内固定术(SSPS-FTTV)治疗,比较两组术后临床疗效。结果(1)A组矢状面Cobb角、VAS评分显著低于B组,伤椎前缘高度比高于B组(P<0.05)。(2)A组内固定失败率4.35%,B组17.39%,A组内固定失败率显著低于B组(P<0.05)。结论在胸腰椎骨折患者的临床治疗中,科学、合理的予以经伤椎置钉短节段椎弓根钉内固定术治疗有显著的临床疗效,在降低内固定失败率、提升矢状面Cobb角、伤椎前缘高度比等与跨伤椎置钉短节段椎弓根钉内固定术相比优势更显著。Objective To investigate the effect of transpedicular fixation with short-segment pedicle screw fixation in patients with thoracolumbar fractures.Methods Twenty-three patients with thoracolumbar fracture admitted to the county hospital were selected for clinical control.Among them,group A was treated with transpedicular screw fixation with short segmental pedicle screw fixation(SSPS-IFTV),group B was treated with short segment transpedicular screw fixation(SSPS-FTTV),and the clinical effects of the two groups were compared.Results(1)The Cobb angle and VAS scores of the sagittal plane in group A were significantly lower than those in group B,and the height ratio of the leading edge of the injured vertebra was higher than that in group B(P<0.05).(2)The failure rate of internal fixation in group A was 4.35%,and that in group B was 17.39%.The fixation rate in group A was significantly lower than that in group B(P<0.05).Conclusion In the clinical treatment of thoracolumbar fracture,it has a significant clinical effect to treat it scientifically and reasonably with short segment pedicle screw fixation through the injured vertebra.Compared with short segment pedicle screw fixation,it has more significant advantages in reducing the failure rate of internal fixation,improving the Cobb angle of sagittal plane and the height ratio of the anterior edge of the injured vertebra.
