左西孟旦对急性心力衰竭患者心功能的影响

摘　　要：目的:探讨左西孟旦注射液治疗急性心力衰竭患者的疗效。方法:选择2017年12月—2018年12月在新疆维吾尔自治区人民医院CCU收治的急性心力衰竭患者70例,将其分为观察组和对照组,每组35例。70例患者均接受常规治疗。对照组给予米力农静脉注射,观察组给予左西孟旦静脉注射。所有患者在入院时植入脉搏指示器持续心输出量监测(PICCO)导管,并测量全心射血分数(GEF)、心指数(CI)、心功能指数(CFI)、脑钠肽(BNP)、左室射血分数(LVEF)等。治疗48 h后再用PICCO测量上述指标。结果:两组患者入院时心功能指标比较差异无统计学意义(P>0.05)。治疗48 h后,两组心功能指标较入院时明显改善(P<0.05),且观察组心功能指标改善程度优于对照组,差异有统计学意义(P<0.05)。结论:左西孟旦能显著改善急性心力衰竭患者的CI,GEF,CFI等心功能指标。Objective:to study the efficacy of leosimendan injection in patients with acute heart failure by monitoring pulse indicator continuous cardiac output(PICCO).Methods:Retrospective analysis was performed on 70 patients with acute heart failure admitted to CCU in people′s hospital of the autonomous region from December 2017 to December 2018.There were 35 cases in the leftsimendan group and 35 cases in the control group.All the 70 patients received conventional treatment,including oxygen inhalation,diuretics,angiotensin converting enzyme inhibitors,and vasodilators.The control group was given intravenous milrinone,and the experimental group was given intravenous levosimendan.PICCO catheter was implanted in all patients at admission,and Cardiac index(CI),global ejection fraction(GEF),cardiac function index(CFI)and brain natriuretic peptide(BNP),left ejection fraction(LVEF)were measured.After observation for 48 hours after treatment,the above indexes were measured by PICCO.Results:there was no significant difference in cardiac function parameters between the two groups at admission(P>0.05).After 48 hours,cardiac function indexes of the left simendan group and the milinone group were improved compared with those at admission(P<0.05).Cardiac function indexes of levosimendan group were significantly improved compared with mirinon group(P<0.05).Conclusion:leosimendan can significantly improve the cardiac function indexes of CI,GEF and CFI in patients with acute heart failure.
