高通量血液透析联合海昆肾喜对糖尿病肾病患者氧化应激及微炎症状态的影响

摘　　要：目的探讨高通量血液透析(HFHD)联合海昆肾喜胶囊对糖尿病肾病患者氧化应激及微炎症状态的影响。方法选取2016年6月~2019年5月于云南省玉溪市人民医院进行血液透析治疗超过6个月糖尿病肾病患者80例,按随机数字表法分为对照组与联合组,每组40例。对照组仅给予HFHD治疗,联合组给予HFHD联合海昆肾喜胶囊治疗。比较两组患者治疗前后肾功能指标[血尿素氮(BUN)、血清肌酐(Scr)、肌酐清除率(Ccr)和24 h尿蛋白定量(UAER)],相关炎性指标[血清白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)及肿瘤坏死因子-α(TNF-α)]和氧化应激水平[丙二醛(MDA)、谷胱甘肽过氧化物酶(GSH-Px)及超氧化物歧化酶(SOD)]的差异。比较两组患者治疗后临床有效率,并分析不良反应发生率。结果与治疗前比较,两组患者治疗后BUN、Scr和UAER显著降低,Ccr显著升高,血清IL-6、hs-CRP、TNF-α和MDA水平均显著降低,GSH-Px和SOD水平均显著升高(P<0.05)。联合组Ccr、GSH-Px和SOD指标均显著高于对照组,BUN、Scr、UAER、IL-6、hs-CRP、TNF-α和MDA水平均显著低于对照组(P<0.05)。联合组临床有效率显著高于对照组(P<0.05)。两组患者均无明显不良反应发生。结论HFHD与海昆肾喜胶囊联合治疗可明显改善糖尿病肾病患者的肾功能,缓解炎性状态,降低氧化应激水平,效果比单一HFHD治疗更佳,推荐临床使用。Objective To investigate the effect of high-flux hemodialysis(HFHD)combined with Haikun Shenxi Capsules on oxidative stress and microinflammation in patients with diabetic nephropathy.Methods A total of 80 patients with diabetic nephropathy who underwent hemodialysis for more than 6 months from June 2016 to May 2019 in People′s Hospital of Yuxi City were enrolled.The patients were divided into the control group and the combination group by random number table method,with 40 cases in each group.The control group was only treated with HFHD,while the combination group was treated with HFHD combined with Haikun Shenxi Capsules.The difference of renal function index[blood urea nitrogen(BUN),serum creatinine(Scr),creatinine clearance(Ccr)and 24 h urine protein quantitation(UAER)],related inflammatory index[Serum levels of interleukin-6(IL-6),high-sensitivity C-reactive protein(hs-CRP)and tumor necrosis factor-α(TNF-α)and oxidative stress level[malondialdehyde(MDA),glycopeptide peroxidase(GSH-Px)and superoxide dismutase(SOD)]between the two groups were compared before and after treatment.The clinical effect of the two groups was compared after treatment,and the incidence of adverse reactions was analyzed.Results Compared with before treatment,BUN,Scr,and UAER in the two groups after treatment were significantly decreased,Ccr was significantly increased,and serum IL-6,hs-CRP,TNF-α,and MDA levels were significantly decreased,GSH-Px and SOD levels were significantly increased(P<0.05).The Ccr,GSH-Px and SOD indexes of the combination group were significantly higher than those of the control group,while the BUN,Scr,UAER,IL-6,hs-CRP,TNF-αand MDA levels were significantly lower than those of the control group(P<0.05).The clinical effective rate in the combination group was significantly better than that of the control group(P<0.05).There was no significant adverse reaction in both groups.Conclusion The combination of HFHD and Haikun Shenxi Capsules can significantly improve the renal function,relieve inflammatory state a
