老年营养风险指数在行放射治疗高龄食管癌患者预后评估中的价值

摘　　要：目的探讨老年营养风险指数(GNRI)对行放射治疗的高龄食管癌患者临床预后的影响。方法回顾性分析2012年1月~2016年12月在浙江省金华市中心医院行根治性放疗的70例老年(年龄≥70岁)食管癌患者的临床资料及生存资料。根据患者GNRI,将其分为低营养风险组(GNRI≥92)和高营养风险组(GNRI<92),比较两组患者一般资料、近期效果和1、2年生存率。结果两组患者血红蛋白、C反应蛋白比较,差异均有统计学意义(均P<0.05)。低营养风险组1、2年生存率显著高于高营养风险组,差异有统计学意义(P<0.05),高GNRI与患者较好的临床预后明显相关。多因素分析提示GNRI、TNM分期、同步化疗与近期效果是影响预后的独立因素(P<0.05)。结论高GNRI提示患者预后较好,老年营养风险指数是行根治性放疗的老年食管癌的独立预后指标。Objective To explore the effect of the geriatric nutrition risk index(GNRI) on clinical prognosis of elderly patients with esophageal cancer undergoing radiotherapy.Methods The clinical data and survival information of 70 elderly patients(ages≥70 years old) with esophageal cancer undergoing radical radiotherapy at Jinhua Municipal Central Hospital from January 2012 to December 2016 were retrospectively analyzed.The patients were divided into low nutrition risk group(GNRI≥92) and high nutrition risk group(GNRI<92) according to GNRI.The general data,treatment response and 1-year,2-year survival rates between the two groups were compared.Results The differences in hemoglobin and C-reactive protein between the two groups were statistically significant differences(all P <0.05).The 1-year and 2-year survival rates of patients in low nutrition risk group were higher than those patients in high nutrition risk group(P <0.05),while high GNRI was significantly associated with better clinical outcomes.Multivariate analysis indicated that GNRI,TNM stage,concurrent chemotherapy and treatment response were independent prognostic factors(P <0.05).Conclusion The patients with high GNRI have good prognosis.GNRI is an effective prognostic index for elderly esophageal cancer patients undergoing radical radiotherapy.
