甘麦大枣汤加减治疗老年股骨粗隆间骨折PFNA内固定术后谵妄的临床研究

摘　　要：目的:对比分析甘麦大枣汤治疗老年股骨粗隆间骨折行股骨近端髓内钉(PFNA)内固定术后谵妄的临床疗效。方法:选取2013年7月-2017年1月由我院收治的因股骨粗隆间骨折行PFNA内固定术治疗后出现谵妄表现的老年患者47例,并按照发病时间先后顺序将其随机分为治疗组(23例)和对照组(24例),治疗组采用甘麦大枣汤治疗,对照组采用氟哌定醇片治疗。结果:治疗组临床疗效总有效率(95.65%)高于对照组(91.67%),两组间比较差异无统计学意义(P>0.05),两组临床疗效总评价表(CGI-SI)分值在治疗后均明显降低,与治疗前比较差异有统计学意义(P<0.01),两组间比较差异无统计学意义(P>0.05);治疗组患者谵妄持续时间与住院时间显著短于对照组患者;治疗组患者经治疗后不良反应发生率显著低于对照组患者。两组在治疗前谵妄分级量表(DRS-R98)评分无统计学差异(P>0.05),治疗后两组的DRS-R98评分均显著降低(P<0.01),但组间比较差异显著性差异(P>0.05)。结论:老年股骨粗隆间骨折PFNA内固定术后谵妄患者服用甘麦大枣汤后临床效果良好,住院时间短、治疗后恢复快,还可临证加减药物,针对性较强,值得临床推广应用。Objective:To analyze the clinical efficacy of the Ganmai Dazao decoction(甘麦大枣汤)on femoral intertrochanteric fractures in the elderly by PFNA internal fixation.Methods:47 cases were selected and divided into the treatment group(23 cases)and control group(24 cases).The treatment group was treated with the Ganmai Dazao decoction and the control group was applied of droperidine alcohol tablets.Results:There was a signi ficant di fference in DRS-R98 score between the two groups after treatment(P<0.05).The total e fficiency in the treatment group(95.65%)was signi ficantly higher than 91.67%in the control group(P>0.05).CGI SI scores in two groups were signi ficantly lower after treatment,with a difference between before and after treatment(P<0.01),without a difference between two groups(P>0.05).The duration of delirium and the time of hospitalization in the treatment group were signi ficantly shorter than the control group,and adverse reactions in the treatment group were signi ficantly lower.Conclusion:The Ganmai Dazao decoction on femoral intertrochanteric fractures in the elderly by PFNA internal fixation was e ffective,and is worthy of clinical application.
