经鼻高流量氧气湿化治疗在老年晚期肺癌合并Ⅰ型呼吸衰竭患者中的应用研究

摘　　要：目的探讨经鼻高流量氧气湿化治疗(HFNC)在老年晚期肺癌合并Ⅰ型呼吸衰竭患者中的应用效果。方法选取老年晚期肺癌合并Ⅰ型呼吸衰竭患者60例,随机分为观察组和对照组,各30例。对照组使用FLEXO无创呼吸机辅助通气治疗,观察组行HFNC治疗。观察2组干预前及干预后2、24 h心率(HR)、动脉血氧分压(PaO2)和血氧饱和度(SpO2)变化。记录2组鼻面部皮肤破损情况。结果干预后2、24 h后,2组HR下降,PaO2、SpO2升高,差异有统计学意义(P<0.05)。观察组各指标改善优于对照组,但差异无统计学意义(P>0.05)。观察组发生鼻面部破损1(3.33%)例,对照组发生鼻面部破损8(26.67%)例,2组比较差异有统计学意义(P<0.05)。结论 HFNC能有效改善老年晚期肺癌合并Ⅰ型呼吸衰竭患者呼吸症状,同时其还能有效避免鼻面部皮肤压疮,患者耐受性较高。Objective To investigate the effect of heated humidified high flow nasal cannula oxygen therapy(HFNC) in elderly patients with advanced lung cancer complicated with type Ⅰ respiratory failure. Methods Totally 60 advanced lung cancer with type Ⅰ respiratory failure elderly patients were randomly divided into two groups, each group has 30 cases. The control group was treated with FLEXO non-invasive ventilator assisted ventilation;the observation group was treated with HFNC. The changes of heart rate(HR), arterial partial pressure of oxygen(PaO2)and oxyhemoglobin saturation(SpO2) in two groups before and after treatment were compared. The damage of facial skin was evaluated. Results The HR, PaO2 and SpO2 were improved in the two groups at 2 and 24 hours after treatment(P<0.05). Patients in the observation group had better HR, PaO2 and SpO2 compared with those in the control group, but with no significant difference(P>0.05). The incidence rate of damage of facial skin in the observation group was lower than that in the control group(3.33%, vs. 26.67%, P<0.05). Conclusion Compared with non-invasive ventilation, HFNC is more suitable for elderly advanced lung cancer patients with type Ⅰ respiratory failure, and associated with lower risk of pressure injuries of facial skin.
