摘　　要：目的:分析内镜下经鼻蝶窦入路行脑垂体瘤切除术后并发颅内感染相关因素,为预防和控制术后颅内感染提供参考性的依据。方法:选取92例2017年2月―2018年2月在我院治疗的脑垂体瘤患者,所有患者均在内镜下经鼻蝶窦入路行脑垂体瘤切除术,对本组患者的临床资料进行回顾性的分析,并对术后颅内感染的相关危险因素进行统计和分析。结果:本组患者中有6例发生术后并发颅内感染,感染率为6.52%。导致患者术后并发颅内感染的危险因素主要有:术中出血量、脑脊液漏、糖尿病、术前应用激素、抗菌药物、垂体功能低下、手术时间>4 h等,差异均存在统计学意义(P<0.05)。结论:有多种因素会导致内镜下经鼻蝶窦入路行脑垂体瘤切除术后患者并发颅内感染,还对患者的预后造成了不良的影响。临床上应加强对术前患者的病情评估,为预防和控制脑垂体瘤切除术后并发颅内感染的发生和发展提供有力的支持。Objective:To analyze the related factors of intracranial infection after endoscopic transsphenoidal approach for pituitary adenoma,and to provide a reference for prevention and control of postoperative intracranial infection.Methods:A total of 92 patients with pituitary tumors who were treated in our hospital from February 2017 to February 2018 were enrolled.All patients underwent endoscopic transsphenoidal approach for pituitary tumor resection.Clinical data of this group of patients were selected and a retrospective analysis was performed and the risk factors associated with postoperative intracranial infection were statistically analyzed.Results:Six patients in this group developed postoperative intracranial infection,with an infection rate of 6.52%.The risk factors for postoperative intracranial infection included intraoperative blood loss,cerebrospinal fluid leakage,diabetes,preoperative application of hormones,antibiotics,hypopituitarism,operative time>4 h,etc.,the differences were statistically significant(P<0.05).Conclusion:There were a variety of factors that could lead to intracranial infection in patients with pituitary adenoma after endoscopic transsphenoidal approach,and it also had a negative impact on the prognosis of patients.Clinical evaluation of preoperative patients should be strengthened to provide strong support for the prevention and control of the occurrence and development of intracranial infections after pituitary adenoma resection.
