摘　　要：目的分析止血愈合海绵在输卵管切开取胚术中的应用价值。方法选取2015年12月~2018年5月赣州市人民医院收治的58例输卵管异位妊娠患者,均采取腹腔镜输卵管切开取胚术治疗,根据患者入院时间的不同分为观察组(28例)和对照组(30例)。对照组术中采用垂体后叶素止血,观察组在对照组基础上加止血愈合海绵。观察两组术中出血量、输卵管切除率、术后异位妊娠发生率的差异;随访1年,观察两组患侧输卵管通畅率、宫内妊娠率、重复异位妊娠率。结果观察组术中出血量少对照组;观察组输卵管切除率、术后异位妊娠发生率低于对照组,差异均有统计学意义(P<0.05)。随访1年,观察组患侧输卵管通畅率、宫内妊娠率高于对照组,异有统计学意义(P<0.05);两组重复异位妊娠率比较,差异无统计学意义(P>0.05)。结论止血愈合海绵在输卵管切开取胚术中的应用能减少术中出血量,降低输卵管切除率,有助于改善患者预后情况,提高输卵管通畅度和宫内妊娠率,值得临床推广使用。Objective To analyze the application value of hemostatic healing sponge in salpingotomy for embryo.Methods A total of 58 patients with tubal ectopic pregnancy admitted to Ganzhou People′s Hospital from December 2015 to May 2018 were selected.All patients were treated by laparoscopic salpingotomy,and were divided into observation group(n=28)and control group(n=30)according to the time of admission.The control group adopted Pituitrin to stop bleeding,and the observation group added hemostatic healing sponge on the basis of the control group.The intraoperative blood loss,fallopian tube resection rate and postoperative ectopic pregnancy rate between the two groups were observed.The tubal patency at the affected side,intrauterine pregnancy rate and repeated ectopic pregnancy rate were followed up for one year.Results The amount of bleeding in the observation group was less than that in the control group(P<0.05).The rate of tubal resection and postoperative ectopic pregnancy in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The patency rate and intrauterine pregnancy rate of the observation group were higher than those of the control group from the one-year follow-up,the differences were statistically significant(P<0.05).There was no significant difference in the repeat ectopic pregnancy rate between the two groups(P>0.05).Conclusion The application of hemostatic healing sponge in salpingotomy for embryo can reduce the amount of intraoperative blood loss,thereby reducing the rate of tubal resection,help to improve the prognosis of patients,improve the patency of fallopian tubes and intrauterine pregnancy rate,which is worthy of clinical promotion and application.
