摘 要:目的:探讨双水平气道正压通气(BiPAP)对慢性阻塞性肺病急性加重(AECOPD)合并Ⅱ型呼吸衰竭患者血气分析及血液流变学的影响。方法:将80例AECOPD合并Ⅱ型呼吸衰竭患者分为治疗组及对照组,各40例。两组均给予常规治疗,治疗组在常规治疗的基础上加用BiPAP治疗。观察两组治疗前后血气指标、血液流变学指标的变化。结果:治疗后两组患者pH、PaO2、SaO2水平较治疗前升高,PaCO2水平较治疗前降低,治疗组改善更明显(均P<0.05或P<0.01);治疗后两组红细胞比容、纤维蛋白原、D-二聚体、血液黏度、全血高切黏度、全血低切黏度水平较低于同组治疗前,且治疗组低于对照组(均P<0.05或P<0.01)。结论:BiPAP结合常规治疗能显著改善AECOPD合并Ⅱ型呼吸衰竭患者缺氧状态,降低血液黏度,改善微循环。Objective:To explore the effect of BiPAP on blood gas analysis and hemorheology in AECOPD patients complicated with type Ⅱ respiratory failure.Methods:Eighty AECOPD patients complicated with type Ⅱ respiratory failure were divided into two groups:treatment group(40 cases)and control group(40 cases).Both groups were given routine treatment,and the treatment group was treated by BiPAP on the basis of routine treatment.The changes of blood gas indexes and hemorheology indexes were observed before and after treatment.Results:After treatment,the pH,PaO2 and SaO2 levels of the two groups were higher than those before treatment,the PaCO2 level was lower than that before treatment,and the improvement of the treatment group was more obvious(all P<0.05 or P<0.01);after treatment,the hematocrit,fibrinogen,D-dimer,blood viscosity,whole blood high shear viscosity and whole blood low shear viscosity were lower than those before treatment,and the reduction of the treatment group was more significant(all P<0.05 or P<0.01).Conclusion:BiPAP combined with routine treatment can significantly improve the hypoxic status,reduce blood viscosity and improve microcirculation in AECOPD patients complicated with type Ⅱ respiratory failure.
