先天性心脏病肺动脉高压介入封堵前后血清血管紧张素转化酶2水平研究

摘　　要：目的 观察先天性心脏病(congenital heart disease,CHD)伴肺动脉高压(pulmonary arterialhypertension,PAH)患者介入封堵手术前后血管紧张素转化酶2(angiotensin-converting enzyme2,ACE2)水平的变化情况.方法 收集2015年3~12月昆明医科大学附属延安医院心血管内科的CHD患者59例,其中CHD肺动脉压力正常组21例(A组);CHD合并轻度PAH组20例(B组);CHD合并中度PAH组18例(C组).介入封堵前后抽取受试者静脉血,应用酶联免疫吸附法(Enzyme-Linked ImmunoSorbent Assay,ELISA)法检测ACE2水平.结果 介入封堵前,C组血清ACE2低于A、B组[(13.11±1.93)U/L比(21.39±1.04)U/L、(16.65±2.39)U/L,P<0.001];介入封堵后,B组血清ACE2较术前稍有升高[(17.01±2.23)U/L比(16.65±2.39)U/L,P=0.09],C组血清ACE2较术前升高[(13.52±2.19)U/L比(13.11±1.93)U/L,P<0.05].结论 CHD伴PAH患者介入治疗后血清ACE2水平升高,ACE2是可能CHD伴PAH患者术后治疗策略选择的潜在生物标志物.Objective To observed the changes of serum angiotensin-converting enzyme2(ACE2)in patients with pulmonary arterial hypertension(PAH)due to congenital heart disease(CHD)before and after intervention closure.Methods From March 2015 to December 2015,59 patients with congenital heart disease were collected in department o f cardiology in Yan'an Affiliated Hospital of Kunming Medical University.The patients with CHD were divided into non-PAH(group A,n=2\),mild PAH(group B,n=20)and moderate PAH(group C,n=18).The serum ACE2 was detected by enzyme-linked immunosorbent assay(ELISA)before and after intervention closure.Results Before intervention closure,the serum ACE2 in group C was significantly lower than that in group A and B[(13.11士1.93)U/L vs.(21.39±1.04)U/L and(16.65土2.39)U/L,P<0.001].After intervention closure,in group B,serum ACE2 was slightly increased[(17.01±2.23)U/L vs.(16.65±2.39)U/L,P=0.09];in group C,the serum ACE2 was increased[(13.52±2.19)U/L vs.(13.11±1.93)U/L,P<0.05].Conclusion The serum ACE2 were increased in the patients with CHD-PAH after interventional closure,that might be used as a potential clinical biomarker to define medical strategies for CHD-PAH after the operation.
