摘 要:目的探讨枸橼酸咖啡因对呼吸暂停早产儿早期肺功能的影响.方法随机收集2015年10月至2018年10月山西省运城市中心医院收治的呼吸暂停早产儿80例,回顾性对照分析其相关临床资料.按所用药物将80例早产儿分为两组,每组40例.枸橼酸咖啡因组给予枸橼酸咖啡因治疗,氨茶碱组则给予氨茶碱治疗.比较两组患儿相关通气指标、用药时间、不良反应情况.结果枸橼酸咖啡因组患儿经鼻持续气道正压通气时间为(134±16)h、有创通气时间为(35±7)h、用氧时间为(140±40)h、用药时间为(191±70)h,均低于氨茶碱组(P均<0.05).枸橼酸咖啡因组患儿TEF25(32.11±0.60)、TEF50(46.51±0.75)、潮气量[(7.52±0.55)ml/kg]、呼吸比(0.68±0.04)、达峰时间比[(25.70±0.77)%]与达峰容积比[(29.66±0.61)%]低于氨茶碱组(P<0.05).氨茶碱组不良反应比例为32.5%(13/40),枸橼酸咖啡因组为47.5(19/40),两组比较差异未见统计学意义(P>0.05).结论枸橼酸咖啡因可改善呼吸暂停早产儿早期肺功能,远期预后较好.Objective To investigate the effects of caffeine citrate on early lung function in preterm infants with apnea.Methods A total of 80 preterm infants with apnea admitted to Yuncheng Central Hospital of Shanxi Province from October 2015 to October 2018 were randomly selected,and the clinical data were retrospectively analyzed.According to the drugs used,80 premature infants were divided into two groups,with 40 casein each group.The citrate caffeine group was given caffeine citrate,and the aminophylline group was given aminophylline.The related ventilation indexes,medication time and adverse reactions were compared between the two groups.Results The nasal continuous positive airway pressure(NCPAP)ventilation time(134±16)h,invasive ventilation time(35±7)h,oxygen time(140±40)h,and medication time(191±70)h in the citrate caffeine group were lower than those in the aminophylline group(P all<0.05).Tidal expiratory flow 25%(TEF25)(32.11±0.60),TEF50(46.51±0.75),tidal volume(7.52±0.55)ml/kg,respiratory ratio(0.68±0.04),peak time ratio(25.70±0.77)and peak volume ratio(29.66±0.61)%of the citrate caffeine group were lower than that of aminophylline group(P<0.05).The proportion of adverse reactions in the aminophylline group was 32.5%(13/40),and that in the caffeine citrate group was 47.5(19/40),and there was no significant difference between the two groups(P>0.05).Conclusions Caffeine citrate treatment has significant effects on early lung function in preterm infants with apnea,and has a good long-term prognosis.
