摘 要:目的研究不同年龄段女性腰椎骨密度(BMD)与BMI及腹部脂肪测量参数的相关性,探讨年龄分层在女性腰椎BMD与肥胖相关研究中的必要性.方法前瞻性收集314例女性腰部定量CT(QCT)数据,以10年为年龄段分为6组,测量其身高、体质量,并计算BMI.数据导入Midways pro QCT分析系统,测量脐水平腹部脂肪面积(FA)、内脏脂肪面积(VFA)、L2~L4椎体BMD,计算腹壁脂肪面积(SFA)及腰椎平均骨密度(vBMD).采用单因素方差分析比较不同年龄段各指标的组间差异;采用Pearson相关分析不同年龄段女性腰椎vBMD与各指标的相关性.结果女性腰椎峰值vBMD出现在26~35岁组,不同年龄组vBMD、FA、VFA差异有显著性(均P<0.05).在≤25岁及26~35岁组,腰椎vBMD与FA(r=-0.5,P=0.042;r=-0.352,P=0.014),特别是SFA呈显著负相关(r=-0.491,P=0.045;r=-0.299,P=0.033),与年龄和BMI无相关性;36~45岁组,腰椎vBMD与各项参数均无相关性;46~55岁及56~65岁组,腰椎vBMD与年龄呈显著负相关(r=-0.265,P=0.003;r=-0.253,P=0.003);56~65岁及≥66岁组,腰椎vBMD与BMI呈显著正相关(r=0.179,P=0.042;r=0.150,P=0.033).结论不同年龄段女性腰椎BMD与BMI及腹部脂肪测量参数之间关系复杂,在相关研究中进行合理的年龄分层十分必要.Objective To study the correlation of bone mineral density(BMD)of lumbar vertebra,the BMI and abdominal fat measurements in women of different age groups,and to explore the necessity of age stratification in the study of the correlation between female BMD and obesity.Methods The data of 314 female quantitative CT(QCT)were collected prospectively.They were divided into 6 groups according to the age of 10 years,the height,weight and BMI were measured.The datas were imported into Midways pro QCT analysis system,to measure the BMD of lumbar 2-4 vertebral body,while the abdominal fat area(FA)and the visceral fat area(VFA)were measured in umbilical axial image.Then the lumbar average BIVID(vBMD)and the surface fat area(SFA)were calculated.One-way ANOVA was used to analyze the differences among different age groups of vBMD,BMI,FA,VFA and SFA,and Pearson test was used to analyze the correlation between vBMD and BMI,FA,VFA,SFA under age stratification.Results The peak of vBMD was presented in the 26-35 age group,and there were significant differences of vBMD,FA and VFA in different age groups(all P<0.05).In groups under≤25 and 26-35,vBMD was negatively correlated with FA(r=-0.5,P=0.042;r=-0.352,P=0.014),especially SFA(r=-0.491,P=0.045;r=-0.299,P=0.033),but no correlation with age and BMI.In group of 36-45,there was no significant correlation between vBMD and other parameters.In groups of 46-55 and 56-65,the vBMD was negatively correlated with age(r=-0.265,P=0.003;r=-0.253,P=0.003);In groups of 56-65 and 66,there was a significant positive correlation between vBMD and BMI(r=0.179,P=0.042;r=0.150,P=0.033).Conclusion The relationship between vBMD,BMI and abdominal fat measurement parameters in women with different age groups is complex and it is essential to undertake reasonable age stratification in relevant studies.
