双源螺旋CT对急性冠状动脉综合征患者冠状动脉斑块性质及左心室功能的评价

摘　　要：目的探讨双源螺旋CT(DSCT)对急性冠状动脉综合征(ACS)患者冠状动脉斑块性质及左心室功能的评价价值.方法选取接受DSCT检查的600例ACS患者为研究对象.采用DSCT检测ACS患者冠状动脉斑块性质及左心室功能[左室舒张末期容积(LVEDV)、左室收缩末期容积(LVESV)、每搏输出量(SV)及左心室射血分数(LVEF)],并与冠状动脉造影(CAG)及超声心动图(ECHO)检测结果进行对比.结果DSCT诊断粥样斑块诊出率、漏诊率及误诊率分别为96.93%、3.07%及4.41%;诊断软斑块的诊出率、漏诊率及误诊率分别为92.86%、7.14%及2.03%.DSCT检测ACS患者LVEDV、LVESV、SV、LVEF与ECHO的结果无明显差异(P>0.05).结论DSCT能同时评估ACS患者的冠状动脉斑块性质及左心室功能,且均有较高的准确性,对ACS的诊断及病情评估具有重要的临床价值.Objective To explore the value of dual-source spiral CT(DSCT)in the coronary plaques properties and left ventricular function in patients with acute coronary syndrome(ACS).Methods 600 patients with ACS who underwent DSCT examination in internal medicine department of were selected for the study.DSCT was used to detect coronary plaques properties and left ventricular function[left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVESV),stroke volume(SV),and left ventricular ejection fraction(LVEF)]in patients with ACS,and the above results were compared with detection results of coronary angiography(CAG)and echocardiography(ECHO).Results The diagnosis rate,missed diagnosis rate and misdiagnosis rate of atherosclerotic plaques diagnosed by DSCT were 96.93%,3.07% and 4.41%,respectively.The diagnosis rate,missed diagnosis rate and misdiagnosis rate of soft plaques were 92.86%,7.14% and 2.03%.respectively.There were no significant differences in the detection of LVEDV,LVESV,SV,LVEF and ECHO of ACS patients by DSCT compared with those by ECHO(P>0.05).Conclusion DSCT can simultaneously evaluate coronary plaques properties and left ventricular function in patients with ACS,and it has high accuracy and has important clinical value in the ACS diagnosis and disease evaluation.
