摘 要:目的分析性别决定区Y框蛋白4(SOX4)和SOX9在晚期宫颈癌新辅助化疗(NAC)前后的表达差异,及其与预后的关系。方法90例初治晚期宫颈癌患者均给予600 mg·m^-2氟尿嘧啶,静脉滴注,第1~5天+75 mg·m^-2顺铂,静脉滴注,第1天。所有患者均治疗2个疗程,每个疗程28 d。根据疗效分为敏感组和不敏感组。用免疫组化法检测宫颈癌组织中SOX4和SOX9蛋白的表达水平,并计算3年总生存率(OS)。用COX回归分析宫颈癌患者预后的独立危险因素。结果NAC后,敏感组和不敏感组的宫颈癌组织中SOX4蛋白阳性率分别为41.67%(20例/48例)和64.29%(27例/42例),SOX9蛋白阳性率分别为35.42%(17例/48例)和57.14%(24例/42例),差异均有统计学意义(均P<0.05)。NAC后,SOX4蛋白阴性和阳性患者的3年OS分别为88.37%(38例/43例)和48.94%(23例/47例),SOX9蛋白阴性和阳性患者的3年OS分别为91.84%(45例/49例)和39.02%(16例/41例),差异均有统计学意义(均P<0.001)。COX多因素分析显示,分化程度、SOX4和SOX9蛋白表达均是影响晚期宫颈癌患者预后的独立危险因素(均P<0.05)。结论晚期宫颈癌患者NAC后SOX4和SOX9蛋白阳性表达率显著降低,低表达患者预后良好,说明二者可能成为判断预后的指标。Objective To analyze the expressions of sex-determining region Y-box protein 4(SOX4)and SOX9 in advanced cervical cancer before and after neoadjuvant chemotherapy(NAC),and explore their relationships with prognosis.Methods Ninety patients with newly advanced cervical cancer were given 600 mg·m^-2 fluorouracil,intravenous infusion,from the first day to the fifth day+75 mg·m^-2 cisplatin,intravenous infusion,the first day.All patients were treated for 2 courses with 28 d per course.According to the clinical efficacy,the patients were divided into sensitive group and insensitive group.The expressions of SOX4 and SOX9 proteins in cervical cancer tissues were detected by immunohistochemistry,and the 3-year overall survival rate(OS)was calculated.The independent risk factors of prognosis in patients with cervical cancer were analyzed by COX regression.Results After NAC,the positive rates of SOX4 protein in cervical cancer tissues of sensitive group and insensitive group were 41.67%(20 cases/48 cases)and 64.29%(27 cases/42 cases),and the positive rates of SOX9 protein were 35.42%(17 cases/48 cases)and 57.14%(24 cases/42 cases),the differences were statistically significant(all P<0.05).After NAC,the 3-year OS of SOX4 protein negative and positive patients were 88.37%(38 cases/43 cases)and 48.94%(23 cases/47 cases),the 3-year OS of SOX9 protein negative and positive patients were 91.84%(45 cases/49 cases)and 39.02%(16 cases/41 cases),the differences were statistically significant(all P<0.001).COX multivariate analysis showed that the degree of differentiation and the expressions of SOX4 and SOX9 protein were independent risk factors affecting the prognosis of patients with advanced cervical cancer.Conclusion The positive expression rates of SOX4 and SOX9 protein in patients with advanced cervical cancer were significantly decreased after NAC.The prognosis of patients with low expression of SOX4 and SOX9 were good and indicated that they might be the prognostic indicators.
