高低剂量低分子肝素联合阿托伐他汀治疗肾病综合征疗效比较

摘　　要：目的 探讨不同剂量低分子肝素联合阿托伐他汀治疗肾病综合征(NS)的临床疗效。方法 选取仙桃市第一人民医院2017年1月至2018年12月收治的NS患者120例,按随机数字表法分为治疗组和对照组,各60例。两组患者均予常规治疗及口服阿托伐他汀钙片,对照组患者加用低剂量低分子肝素,治疗组患者加用高剂量低分子肝素。结果 治疗组有效率为93.33%,显著高于对照组的76.67%(P<0.05);与治疗前比较,两组患者治疗后的血尿素氮(BUN)、24 h尿蛋白(24hUP)、血肌酐(SCr)、三酰甘油(TG)、总胆固醇(TC)、血浆黏度、红细胞比容均显著降低,凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)均显著延长,治疗组患者上述指标改善情况显著优于对照组(P<0.05);治疗组与对照组不良反应发生率相当(11.67%比10.00%,P>0.05)。结论 与低剂量相比,高剂量低分子肝素联合阿托伐他汀治疗NS,改善肾功能、降低蛋白尿及血脂水平、促进血流循环的疗效更好。Objective To investigate the clinical efficacy of different doses of low molecular weight heparin(LMWH)combined with atorvastatin on nephrotic syndrome(NS).Methods A total of 120 NS patients who were admitted to the hospital from January 2017 to December 2018 were selected and divided into the treatment group and control group according to the random number table method,60 cases in each group.The control group was given low-dose LMWH and atorvastatin calcium,while treatment group was given with high-dose LMWH and atorvastatin calcium.Both groups were continuously treated for 4 months.Results The effective rate in the treatment group was 93.33%,which was significantly higher than 76.67%in the control group(P<0.05).Compared with before treatment,the blood urea nitrogen(BUN),24 h urine protein(24 hUP),serum creatinine(SCr),triacylglycerol(TG),total cholesterol(TC),plasma viscosity,and hematocrit were significantly reduced,while the prothrombin time(PT),activated partial thromboplastin time(APTT)were significantly prolonged;the improvement of the above indicators in the treatment group were significantly better than those in the control group(P<0.05);the incidence of adverse reactions was similar between the two groups(11.67%vs.10.00%,P>0.05).Conclusion High-dose LMWH combined with atorvastatin in treatment of NS can significantly improve renal function,reduce proteinuria and blood lipid levels,and promote blood circulation.
