摘　　要：目的研究生存蛋白(Survivin)在膀胱癌患者尿液脱落细胞中的表达及临床相关性。方法本文选取2016年9月-2018年9月在我院就诊的膀胱癌患者50例,分为膀胱癌组,选取同时期在我院就诊的膀胱良性病变患者20例,分为良性组,选取同一时期在我院健康体检的志愿者20例,分为正常组。尿液脱落细胞DNA提取,实时荧光定量PCR检测Survivin在尿液脱落细胞中的表达。结果膀胱癌组尿液脱落细胞中Survivin表达高于正常组和良性组,良性组尿液脱落细胞中Survivin表达高于正常组,具有统计学差异(P<0.05)。有肌层浸润患者Survivin表达89.29%高于无肌层浸润患者54.55%,具有统计学差异(P<0.05)。T分期≥T2期患者Survivin表达86.67%高于T分期<T2期患者50.00%,具有统计学差异(P<0.05)。病理分级G2-G3级患者Survivin表达88.89%高于病理分级G1级患者47.83%,具有统计学差异(P<0.05)。结论 Survivin在膀胱癌患者尿液脱落细胞中显示高表达,其表达水平与患者肌层浸润、T分期、病理分级有关,参与膀胱癌发展。Objective To study the expression of Survivin in urine exfoliated cells of bladder cancer patients and its clinical correlation.Methods Fifty patients with bladder cancer who were treated in our hospital from September 2016 to September 2018 were divided into bladder cancer group.Twenty patients with benign bladder lesions who were treated in our hospital at the same time were selected and divided into benign group.Twenty volunteers who underwent physical examination in our hospital at the same time were selected and divided into normal group.DNA was extracted from exfoliated urine cells and the expression of Survivin in exfoliated urine cells was detected by real-time fluorescence quantitative PCR.Results The expression of Survivin in exfoliated urine cells of bladder cancer group was higher than that of normal group and benign group.The expression of Survivin in exfoliated urine cells of benign group was higher than that of normal group,with statistical difference(P<0.05).Survivin expression in patients with myometrial invasion was 89.29% higher than 54.55%in patients without myometrial invasion(P<0.05).Survivin expression in patients with T stage> T2 was 86.67% higher than that in patients with T stage< T2 50.00%,with statistical difference(P<0.05).Survivin expression in patients with pathological grade G2-G3 was 88.89% higher than 47.83%in patients with pathological grade G1(P<0.05).Conclusion Survivin is highly expressed in urine exfoliated cells of patients with bladder cancer.Its expression level is related to muscle infiltration,T stage and pathological grading,and it is involved in the development of bladder cancer.
