摘　　要：【目的】探讨同型半胱氨酸(Hey)、脂蛋白a、肝型脂肪酸结合蛋白(L-FABP)在糖尿病肾病(DN)早期肾损伤中的变化及其临床意义。【方法】本院收治的60例2型糖尿病患者(T2DM组)、60例2型糖尿病伴微量白蛋白尿患者(MA组)、60例2型糖尿病肾损伤达到临床期患者(CN组)及60例健康体检者(对照组)为研究对象;比较四组研究对象的Hey、脂蛋白a、L-FABP水平及肾功能指标;并采用线性相关法探讨Hey、脂蛋白a、L-FABP水平与患者肾功能指标变化的关系。【结果】T2DM组、MA组及CN组的Hey、脂蛋白a、L-FABP水平均显著高于对照组(P<0.05),MA组及CN组的Hey、脂蛋白a、L-FABP水平显著高于T2DM组(P<0.05),CN组的Hey、脂蛋白a、L-FABP水平显著高于MA组(P<0.05);CN组的血尿素氮(BUN)、血肌酐(SCr)水平显著高于T2DM组、MA组及对照组(P<0.05),MA组、CN组的尿微量白蛋白排泄率(UAER)值高于T2DM组(P<0.05),CN组的UAER值高于MA组(P<0.05);UAER范围>20畔/min的T2DM患者的Hey、脂蛋白a、L-FABP水平与SCr、UAER呈显著的正相关关系(P<0.05)。【结论】DN患者Hey、脂蛋白a、L-FABP水平显著地升高,并且与患者肾功能损害和病情程度具有相关性。【Objective】To investigate the changes and value of homocysteine(Hey),lipoprotein a and liver fatty acid binding protein(L-FABP)in early renal injury of diabetic nephropathy(DN).【Methods】The study contained patients with type 2 diabetes mellitus(T2DM group,n=60),type 2 diabetes mellitus with microalbum inuria(MA group,n=60),type 2 diabetic kidney injury(CN group,n=60)and healthy volunteers as the control group(n=60).The levels of Hey,lipoprotein a,L-FABP and renal function were compared in the four groups.The relationship of levels of Hey,lipoprotein a and L-FABP with changes in renal function of patients was investigated by a linear correlation m ethod.【Results】The levels of Hey,lipoprotein a and L-FABP in the T2DM group,MA group and CN group were significantly higher than those in the control group(P<0.05).Hey,lipoprotein a and L-FABP levels in the MA group and CN group were higher than those in the T2DM group.The levels of Hey,lipoprotein a and L-FABP in the CN group was significantly higher than those in the MA group(P<0.05).The levels of BUN and SCr in the CN group were significantly higher than those in the T2DM group,the MA group and the control group(P<0.05).The UAER values of the MA group and the CN group were higher than those of the T2DM group(P<0.05);and the UAER value of the CN group was higher than that of the MA group(P<0.05);The levels of Hey,lipoprotein a and L-FABP in the T2DM patients with>20μg/m in were significantly positively correlated with SCr and UAER(P<0.05).【Conclusion】The levels of Hey,lipoprotein a,and L-FABP in patients with DN were significantly elevated and closely correlated with renal dysfunction in patients.
