摘　　要：目的:探讨临床药师对特殊人群应用华法林发生意外情况时的处理和监护.方法:评估药物基因组学、食物和药物相互作用,根据指南制定脑梗死溶栓的个体化建议.结果:通过实施药学监护,并对患者进行用药教育,治疗取得了较好结果.结论:临床药师参与华法林抗凝治疗的全程化药学服务,有助于患者发生意外情况的处理和监护及预后的改善.Objective:To discuss the treatment and monitoring of clinical pharmacists on the occurrence of unexpected situation when the use of warfarin in special population.Methods:A recommendation of individualized treatment for the thrombolysis of cerebral infarction was proposed based on the assessment of pharmacogenomics,food and drug interactions and the guidelines.Results:The treatment achieved good results by implementing pharmacological monitoring and medication education to the patient.Conclusion:Clinical pharmacists should participate in the full-scale pharmacy service of warfarin anticoagulant therapy,which is helpful for the treatment and monitoring of patients with unexpected conditions and the prognosis improvement.
