基于肠道菌群从脾论治多囊卵巢综合征探讨

摘　　要：通过分析多囊卵巢综合征与肠道菌群结构的关系,认为肠道菌群失调导致的胰岛素抵抗是多囊卵巢综合征发病的重要机制,基于肠道菌群,从脾的生理功能及健脾类方药改善多囊卵巢综合征的临床症状等方面,探讨以脾为中心论治多囊卵巢综合征的可行性。肠道菌群之增强机体免疫及促进机体消化、吸收、排泄功能与中医脾脏的抵御外邪、运化水谷精微等生理功能相似,脾失健运形成痰湿是多囊卵巢综合征发病的重要基础。中医经典健脾除湿方如四君子汤、参苓白术散能调节肠道菌群结构,有效改善多囊卵巢综合征的胰岛素抵抗和卵巢多囊样改变。故多囊卵巢综合征治疗应以健脾除湿化痰药物为主,兼予补肾填精、疏肝解郁、活血化瘀类药物。From the analysis of the relationship between polycystic ovary syndrome(PCOS)and the structure of intestinal flora,it is considered that insulin resistance caused by intestinal flora imbalance is an important mechanism for the development of PCOS.Based on the intestinal flora,from the physiological functions of the spleen and the effects of formulas for invigorating spleen to improve the clinical symptoms of PCOS,the treatment of PCOS based on the spleen is discussed.Meanwhile,the function of intestinal flora to enhance immunity and promote digestion,absorption and excretion of the body is similar to that of spleen in traditional Chinese medicine to resist external pathogen and transport and transform water and cereal essence.Phlegm and dampness caused by dysfunction of spleen in transportation is the important basis for PCOS.The classical traditional Chinese formulas,such as Sijunzi Decoction(四君子汤)and Shenling Baizhu Powder(参苓白术散)can regulate the structure of intestinal flora;effectively improve the insulin resistance state of PCOS and the polycystic change of ovary.Therefore,in the treatment of PCOS,Chinese medicine for invigorating spleen and removing dampness and phlegm are mainly used,combined with herbs for invigorating kidney and supplementing essence,soothing liver and relieving depression,promoting blood circulation and removing blood stasis.
