摘 要:目的观察微创脊柱手术治疗胸腰段Chance骨折的临床疗效。方法选取2017年4月至2019年4月期间我院收治的胸腰段Chance骨折患者50例,随机分为两组各25例。对照组行开放性手术治疗,试验组行微创脊柱手术治疗,比较两组的手术指标和治疗效果。结果试验组的手术时间、住院时间显著短于对照组,术中出血量显著少于对照组,椎体高度丢失显著低于对照组,差异均具有统计学意义(P均<0.05);两组的术后Cobb角比较,差异无统计学意义(P>0.05)。术前,两组患者的生活质量评分及伤椎前缘高度无显著差异(P>0.05);术后,试验组患者的生活质量评分显著高于对照组(P<0.05),两组的伤椎前缘高度无统计学差异(P>0.05)。结论微创脊柱手术治疗胸腰段Chance骨折的疗效优于开放性手术,可有效恢复患者椎体高度丢失及伤椎前缘高度,提升患者的生活质量。Objective To observe the clinical efficacy of minimally invasive spinal surgery in the treatment of thoracolumbar Chance fracture.Methods From April 2017 to April 2019, 50 patients with thoracolumbar Chance fracture admitted to our hospital were selected and randomly divided into two groups, with 25 cases in each group. The control group was treated with open surgery, and the experimental group was treated with minimally invasive spinal surgery. The surgical indicators and curative effects of the two groups were compared.Results The operation time and hospitalization time of experimental group were significantly shorter than those of control group, the amount of intraoperative bleeding was significantly less than that of control group, and the loss of vertebral height was significantly lower than that of control group(P<0.05);No statistically significant difference was found in postoperative Cobb angle between two groups(P>0.05). Before operation, no significant difference was found in the quality of life score and the height of anterior edge of injured vertebrae between two groups(P>0.05);After operation, the quality of life score of experimental group was significantly higher than that of control group(P<0.05), and no significant difference was found in the height of anterior edge of injured vertebrae between two groups(P>0.05).Conclusions Minimally invasive spine surgery has better efficacy than open surgery in the treatment of thoracolumbar Chance fracture, which can effectively recover the loss of vertebral height and the height of anterior edge of injured vertebrae, and improve the quality of life of patients.
