血透患者血浆HCY和叶酸、VitB12浓度关系分析及干预研究

摘　　要：目的探讨血透患者血浆HCY和叶酸、VitB12浓度的关系及干预效果。方法选取透龄大于3个月的维持性血液透析患者280例,检测其血浆HCY、叶酸及VitB12浓度并进行分析。将其中60例HCY超过正常值两倍的患者随机分成两组,试验组予叶酸口服治疗1个月,对照组不予叶酸治疗,1个月后比较两组的HCY浓度及治疗效果。结果 280例血透患者中,有256例HCY浓度升高,其中11例叶酸浓度低于正常,余者叶酸浓度都在正常范围内;有232例VitB12浓度升高。相关分析显示,血浆HCY与叶酸、VitB12浓度无相关性(r=-0.023、0.003, P均>0.05)。治疗1个月后,试验组的HCY浓度显著低于对照组(P<0.05)。结论血透患者的血浆HCY与叶酸、VitB12浓度无相关性,口服叶酸治疗能明显降低血透患者的血浆HCY浓度。Objective To explore the relationship of plasma HCY with concentrations of folic acid and Vit B12 in hemodialysis patients and the effect of intervention.Methods The concentrations of plasma HCY, folic acid and VitB12 were detected and analyzed in280 patients with maintenance hemodialysis over 3 months. 60 patients with HCY more than twice the normal value were randomly divided into two groups. The experimental group was treated with folic acid orally for 1 month, while the control group did not receive folic acid.After 1 month, the HCY concentration and the treatment effect were compared between two groups.Results Among 280 hemodialysis patients, 256 cases had increased HCY concentration, of which 11 cases had folic acid concentration lower than normal, and the rest were in the normal range;232 cases had increased VitB12 concentration. Correlation analysis showed that there was no correlation between HCY and concentrations of serum folic acid and VitB12(r=-0.023, 0.003;P>0.05). After 1 month of treatment, the HCY concentration of experimental group was significantly lower than that of control group(P<0.05).Conclusions Plasma HCY has no correlation with concentrations of folic acid and VitB12. Oral folic acid treatment can significantly reduce the plasma HCY concentration in hemodialysis patients.
