含漱清热凉血方对低温等离子切除扁桃体术后炎症反应的影响

摘　　要：目的观察含漱中药煎剂清热凉血方对低温等离子扁桃体切除术后炎症指标及咽部疼痛的影响。方法选取40例慢性扁桃体炎(chronic tonsillitis,CTS)患者,分为治疗组20例,对照组20例。两组患者均在全身麻醉下,由同一位经验丰富的主任医师行低温等离子射频消融切除双侧扁桃体治疗。治疗组从术后第1天起,每天分两次含漱中药煎剂,对照组不使用任何漱口治疗。分别在术后第1天、术后第5天记录患者血液白细胞计数(white blood cell,WBC)、中性粒细胞百分比(neutrophil%,N%)数值。采用咽痛视觉模拟评估法(visual analogue score,VAS)评估患者术后咽部疼痛情况。结果炎症指标:术后第1天,两组间WBC、N%经比较无统计学差异(P>0.05);术后第5天,两组间WBC、N%经比较无统计学差异(P>0.05)。咽痛评分:术后第1天,两组间咽痛评分经比较无统计学差异(P>0.05);术后第5天,两组间比较有统计学差异(P<0.05),提示含漱中药煎剂可以降低咽痛评分。结论含漱中药煎剂清热凉血方对扁桃体术后炎症指标WBC、N%的变化无明显影响,但是可以明显缓解术后咽痛,疗效肯定,使用方便,值得临床推广。Objective To observe the effects of gargling with removing heat and cooling blood decotion after tonsillectomy through low-temperature plasma radiofrequency ablation on inflammatory marker and pharyngeal pain.Methods 40 cases of patients with chronic tonsillitis(CTS)were divided into 2 groups,20 cases of treatment group and 20 cases of control group.All patients received bilateral tonsillectomy through low-temperature plasma radiofrequency ablation by the same experienced archiater under systemic intravenous anesthesia.Since the first day after operation,the treatment group took a dose of gargle twice a day,while the control group received no other gargle treatments.The data of white blood cell(WBC)and neutrophil%(N%)were recorded at postoperation day1 and day5.All postoperative patients were assessed about pharyngeal pain according to self-designed visual analogue score(VAS)method.Results Inflammatory marker:At the postoperation day1 and day5,there was no statistical difference on WBC and N%between the treatment group and control group(P>0.05).Pharyngeal pain VAS:At the postoperation day1,there was no statistical difference between the two groups(P>0.05).At the postoperation day5,there was statistical difference between 2 groups(P<0.05),inferring that gargling with removing heat and cooling blood decotion could decrease pharyngeal pain score.Conclusion Gargling with removing heat and cooling blood decotion after bilateral tonsillectomy by low-temperature plasma radiofrequency ablation has no significant effect on the changes of inflammatory markers(WBC and N%),but it is effective on relieving postoperative pharyngeal pain.Gargling with chinese herbs which has definite therapeutic effects and simple operation mode deserves to be promoted clinically.
