乳晕交叉真皮、皮下组织瓣在防止乳头凹陷术后复发的临床应用

摘　　要：目的 为防止乳头凹陷术后复发,介绍一种矫治乳头凹陷的新方法。方法 乳头上、下方各形成一个菱形真皮、皮下组织瓣,通过乳头下纵向纤维束之间的隧道交叉并固定到对侧的皮下组织,以起到支撑乳头,阻止回缩的目的。结果 2010年 1月至 2019年 7月治疗先天性乳头凹陷患者 18 例,均获得满意效果,本组患者术后切口均I期愈合,无乳头血运障碍发生,感觉正常。随访1~5年,乳头高度及外形理想,疤痕不明显,无一例复发。结论该手术方法是矫治先天性乳头凹陷的一种理想方法,操作简便,效果可靠,切口瘢痕少,乳头形态好,感觉良好,可有效防止乳头凹陷复发,值得推广应用。Objective In order to prevent the recurrence of nipple depression after operation,a new method for correcting nipple depression is introduced.Methods A rhomboid dermis and subcutaneous tissue flap were formed above and below the nipple,which were crossed through the tunnel between the longitudinal fiber bundles under the nipple and fixed to the subcutaneous tissue on the opposite side,so as to support the nipple and prevent retraction.Results From January 2010 to July 2019,18 patients with congenital nipple depression were treated with satisfactory results.all the incisions in this group healed in the first stage,there was no disturbance of nipple blood circulation,and the feeling was normal.Conclusion This operation is an ideal method for the correction of congenital nipple depression,which has the advantages of simple operation,reliable effect,less incision scar,good nipple shape and good feeling,and can effectively prevent the recurrence of nipple depression,so it is worth popularizing and applying.
