摘　　要：目的探究鼻唇沟皮瓣对鼻唇部畸形缺损的修复作用。方法选取鼻唇部畸形缺损患者28例为研究对象,患者缺损畸形部位包括鼻尖、鼻翼、唇部,给予患者鼻唇沟皮瓣修复术进行修复。观察患者住院时间、术后恢复时间;观察患者术后6个月时瘢痕增生程度、鼻部形态(小柱小叶比、鼻背长度、小柱上唇角)、心理状态[口腔颌面部躯体变形障碍自评量表(OFBDDSS)]情况。结果28例患者术后皮瓣均为I期存活,24例患者恢复良好,无瘢痕;2例患者为轻度瘢痕,2例患者为重度瘢痕,恢复良好率为92.86%;患者鼻部形态恢复良好,平均小柱小叶比(1.89±0.47)、鼻背长度(1.06±0.12)cm、小柱上唇角(94.15±6.43)°。OFBDDSS量表Ⅰ~Ⅳ量表评分为(7.87±1.46)分、(8.06±1.03)分、(11.35±1.43)分、(5.93±0.46)分。结论鼻唇沟皮瓣修复术可利于鼻唇部畸形缺损患者术后恢复,瘢痕恢复较佳,还可显著改善患者鼻结构,患者对修复效果更易取得满意心理。Objective To explore the repair effects of nasolabial flaps on nasolabial deformities and defects.Methods Twenty-eight patients with nasolabial deformities and defects were selected as the research subjects.The defect and deformity areas of the patients included the nasal tip,nasal wing and lip,and the patients were repaired with nasolabial flaps.The hospital stay and postoperative recovery time were observed.The degree of scar hyperplasia,nasal shape(pillaret-to-lobule ratio,nasal dorsal length,pillaret upper lip angle)and psychological status[oromaxillo-facial body dysmorphic disorder self-rating scale(OFBDDSS)]were observed at 6 months after surgery.Results 28 patients were with stage I survival of postoperative flaps,and 24 patients were with good recovery and without scars.2 patients were mild scars and 2 patients were severe scars,and the good recovery rate was 92.86%.The average columella lobule ratio(1.89±0.47),the length of the back of nose(1.06±0.12)cm,and the upper lip angle of columella(94.15±6.43)°were observed.The scores of ofbddss I-IV were(7.87±1.46),(8.06±1.03),(11.35±1.43),(5.93±0.46).Conclusion Nasolabial flap repair can be beneficial for postoperative recovery and good scar recovery of patients with nasolabial deformities and defects.It can also significantly improve the nasal structure of patients,and it is easier for patients to obtain satisfactory repair effects.
