摘 要:目的研究慢性乙型肝炎患者白细胞介素(IL)-17、IL-6、转化生长因子(TGF)-β、基质金属蛋白酶(MMP)-2表达水平,分析这些指标与疾病严重程度的关系,以及复方甘草酸苷对慢性乙型肝炎患者血清IL-17、IL-6、TGF-β、MMP-2表达的影响。方法测定慢性乙型肝炎患者(55例)及健康对照者(20例)IL-17、IL-6、TGF-β、MMP-2的表达水平,分析以上细胞因子与疾病严重程度的关系;测定加用复方甘草酸苷治疗的慢性乙型肝炎患者(治疗组)及采用常规治疗的慢性乙型肝炎患者(对照组)治疗前后肝功能指标的变化及IL-17、IL-6、TGF-β、MMP-2表达水平。结果慢性乙型肝炎患者血清IL-17、IL-6、MMP-2、TGF-β水平均高于健康对照者(P<0.05),且随着疾病严重程度的增加,IL-17、IL-6、MMP-2、TGF-β水平升高(P<0.05);IL-17、IL-6、TGF-β、MMP-2表达水平与疾病严重程度呈正相关(P<0.05)。治疗2周后,治疗组患者ALT、TBIL降低程度大于对照组(P<0.05),IL-17、IL-6、TGF-β水平降低程度也均大于对照组(P<0.05)。结论IL-17、IL-6、MMP-2、TGF-β参与了慢性乙型肝炎的发生、发展,复方甘草酸苷可能通过降低患者血清IL-17、IL-6、TGF-β水平参与免疫调节,发挥抗炎、保肝作用。Objective To study the expression levels of interleukin(IL)-17,IL-6,transforming growth factor(TGF)-β and matrix metalloproteinase(MMP)-2 in the patients with chronic hepatitis B and their relationship with disease severity,and the influence of Compound Glycyrrhizin on serum IL-17,IL-6,TGF-β and MMP-2 expression levels in the patients with chronic hepatitis B.Methods The expression levels of IL-17,IL-6,TGF-β and MMP-2 in the patients with chronic hepatitis B(n=55)and healthy controls(n=20)were detected and the relationship between the above cytokines and disease severity was analyzed;the expression levels of IL-17,IL-6,TGF-β,MMP-2 and liver function indexes in the patients with chronic hepatitis B before and after adding Compound Glycyrrhizin treatment(treatment group)and control group adopting the routine treatment(control group)were detected.Results The levels of serum IL-17,IL-6,MMP-2 and TGF-βin the patients with chronic hepatitis B were higher than those in the healthy controls(P<0.05),which were increased with the disease severity increase(P<0.05),the expression levels of IL-17,IL-6,TGF-β and MMP-2 were positively correlated with the disease severity(P<0.05).The decrease degrees of ALT and TBIL after 2-week treatment in the treatment group were greater than those in the control group(P<0.05),and the decrease degrees of IL-17,IL-6 and TGF-βlevels in the treatment group were greater than those in the control group(P<0.05).Conclusion IL-17,IL-6,MMP-2 and TGF-β are involved in the occurrence and development of chronic hepatitis B.Compound Glycyrrhizin may play an anti-inflammatory and hepatoprotective effect by reducing the expression of serum IL-17,IL-6 and TGF-β and participating in the immune regulation in the patients.
