摘 要:目的观察急性B淋巴细胞白血病(B-ALL)患儿外周血单个核细胞WT1基因表达变化,并探讨其表达变化与患儿临床病理特征和预后的关系。方法选择B-ALL患儿83例(观察组)、择期手术的非B-ALL患儿40例(对照组),采集外周静脉血,提取单个核细胞,采用RT-qPCR法检测WT1基因表达。比较两组外周血单个核细胞WT1基因表达差异,并分析外周血单个核细胞WT1基因表达与B-ALL患儿临床病理特征和预后的关系。结果观察组外周血单个核细胞WT1基因相对表达量明显高于对照组(t=19.231,P<0.01)。外周血单个核细胞WT1基因相对表达量与B-ALL患儿年龄、流式亚型、危险程度有关(P均<0.05),而与B-ALL患儿性别、染色体核型、发病状态和WBC计数无关(P均>0.05)。以外周血单个核细胞WT1基因相对表达量的均数7.16为临界值,将B-ALL患儿分为WT1基因高表达者42例、低表达者41例。WT1基因高表达者5年总生存率低于WT1基因低表达者(χ^2=4.142,P<0.05)。结论B-ALL患儿外周血单个核细胞WT1基因表达升高,其异常表达与患儿年龄、流式亚型、危险程度和预后有关。Objective To observe the changes of WT1 gene expression in the peripheral blood mononuclear cells of children with acute B-lymphoblastic leukemia(B-ALL),and to explore the relationship between the changes of WT1 gene expression and the clinicopathological characteristics and prognosis of children.Methods Eighty-three children with B-ALL(observation group)and 40 children who underwent elective surgery(control group)were selected.We collected their peripheral venous blood,extracted mononuclear cells,and detected WT1 gene expression by RT-qPCR.The differences of WT1 gene expression in peripheral blood mononuclear cells of the two groups were compared,and the relationships of WT1 gene expression in peripheral blood mononuclear cells with the clinicopathological characteristics and prognosis of B-ALL children were analyzed.Results The relative expression of WT1 gene in the peripheral blood mononuclear cells of the observation group was significantly higher than that of the control group(t=19.231,P<0.01).The relative expression of WT1 gene in the peripheral blood mononuclear cells was related to the age,flow subtype and risk degree of B-ALL children(all P<0.05),but not to the gender,karyotype,disease state or WBC count of B-ALL children(all P>0.05).We took the mean relative expression level of WT1 gene in peripheral blood mononuclear cells 7.16 as the critical value,and then the B-ALL children were divided into 42 patients with high expression level of WT1 gene and 41 patients with low expression level.The 5-year overall survival rate of those with high WT1 gene expression was lower than that of patients with low WT1 gene expression(χ^2=4.142,P<0.05).Conclusion WT1 gene expression increases in the peripheral blood mononuclear cells of children with B-ALL,and its abnormal expression is related to the age,flow subtype,risk degree and prognosis.
