电针联合健脾祛湿方治疗2型糖尿病合并非酒精性脂肪肝患者的临床研究

摘　　要：目的:观察电针治疗联合健脾祛湿方对2型糖尿病(T2DM)合并非酒精性脂肪肝(NAFLD)患者的临床疗效。方法:选取2014年7月至2017年3月间来我院就诊的140名T2DM合并NAFLD患者,分为对照组(68例)及观察组(72例)。基础降糖治疗的同时给予2组健脾祛湿方治疗,观察组另取肝俞、足三里、丰隆、太冲、脾俞等穴位电针治疗,24周后比较2组患者的总有效率、血糖、胰岛素抵抗指数、血脂、肝功能、糖化血红蛋白水平。结果:观察组的总有效率高于对照组。治疗后2组患者血糖、空腹胰岛素、胰岛素抵抗指数、糖化血红蛋白、总胆固醇、甘油三酯、低密度脂蛋白胆固醇、丙氨酸氨基转移酶、天冬氨酸转氨酶、γ-谷氨酰转肽酶显著下降,且观察组低于对照组;高密度脂蛋白胆固醇显著上升,且观察组高于对照组。结论:电针联合健脾祛湿方治疗T2DM合并NAFLD效果显著,能纠正患者糖脂代谢紊乱,保护胰岛功能并显著改善其血脂水平和肝脏功能。Objective:To observe the clinical effect of electroacupuncture combined with spleen-strengthening clearing damp prescription on patients with type 2 diabetes mellitus(T2 DM)complicated with non-alcoholic fatty liver disease(NAFLD).Methods:140 patients with T2 DM and NAFLD who came to our hospital from July 2014 to March 2017 were selected.The patients were divided into control group(68 cases)and observation group(72 cases).All patients received basic hypoglycemic treatment,while the control group was treated with spleen-strengthening clearing damp prescription.The observation group was treated with electro-acupuncture at Ganshu,Zusanli,Fenglong,Taichong and Pishu points on the basis of the control group.After 24 weeks,the total effective rate of two groups of patients were compared,the levels of blood sugar,insulin resistance index,blood lipid,liver function and glycosylated hemoglobin before and after treatment were compared to the two groups.Results:The total effective rate of the observation group was higher than the control group.After treatment,the fasting blood glucose,2-hour postprandial blood glucose,fasting insulin,insulin resistance index,glycosylated hemoglobin,total cholesterol,triglyceride,low density lipoprotein cholesterol,alanine aminotransferase,aspartate aminotransferase and gamma-glutamyltransferase in the two groups decreased significantly,and the observation group was lower than the control group.After treatment,the high density lipoprotein cholesterol in both groups was significantly increased,and the observation group was higher than the control group.Conclusion:Electroacupuncture combined with spleen-strengthening clearing damp prescription is effective in treating T2 DM with NAFLD.It can correct the disorder of glucose and lipid metabolism,protect the function of islets and significantly improve the blood lipid level and liver function.
