丁元庆辨治暑咳迁延不愈经验

摘　　要：针对临床辨治暑咳过用苦寒苦燥药致暑咳迁延不愈的现状,总结丁元庆教授辨治该病的经验。丁元庆认为暑咳初期在肺,暑热或暑湿犯肺,肺气壅遏不能宣降而致咳,失治误治或患者素体气阴亏虚,暑邪可循经伤脾胃气阴、灼肝肾阴精,演化为内伤咳嗽。临床可分暑咳在肺、在脾胃、在肝肾辨治,指出咳痰急迫量多,痰色虽为白色亦应辨为热证,治暑用药宜辛平、甘润、苦降,还可适时应用健脾养阴甘温药,着重论述其辨治暑咳时对清半夏、石膏、桑叶、白扁豆、熟地黄的独到认识,并举暑咳阴伤病案进一步论证其辨治暑咳迁延不愈的诊治特色。Summer-heat cough treated with bitter-cold drugs or bitter-dryness drugs can cause the disease reoccurring and postponing,the diagnosis and treatment of the disease of Prof.DING Yuanqing was summarized.It is pointed out that summer-heat effected on lung in early period.Summer-heat or summer-damp can obstruct the lung-qi and cause lung qi failing to dispersing and descending.Pathogenic summer-heat can also damage qi-yin of spleen and stomach and waste the essence of liver and kidney along the main and collateral channels due to mistreatment or deficiency of qi and yin of the patients,then exogenous cough can evolve into endogenous cough.Summer-heat cough can be diagnosed and treated from lung,spleen and stomach,liver and kidney.Prof.DING pointed out cough with breathlessness and a large quantity of white sputum should be diagnosed as heat syndrome.Pungent-cool,sweet-moistening,bitter-dispersing drugs were used to treat summer-heat cough.Invigorating the spleen and nourishing yin drugs with sweet in taste and warm in nature can also be applied to use.The characteristics of drugs,such as Qingbanxia(PINELLIAE RHIZOMA PRAEPARATUMCUM ALUMINE),Shigao(GYPSUM IBROSUM),Sangye(MORI FOLIUM),Baibiandou(LABLAB SEMEN ALBUM)and Shudihuang(REHMANNIAE RADIX PRAEPARATA),were discussed in detail.A cases of summer-heat cough with yin detriment was analyzed to further confirm the diagnosis and treatment Prof.Ding in treating the disease.
