肺炎患者血清Clara细胞分泌蛋白16与机械通气预后的相关性分析

摘　　要：目的分析肺炎患者血清Clara细胞分泌蛋白16(CC16)与机械通气预后的相关性。方法选择在我院住院并行机械通气治疗的肺炎患者48例为观察组,选择同期进行健康体检者50例为对照组。比较两组临床指标,并分析观察组临床疗效与指标的相关性。结果观察组的治疗总有效率为83.33%。治疗后72 h,观察组的FEV1/FVC、FEV1、CC16水平、APACHEⅡ、CPIS评分均优于治疗前及治疗后24 h(P<0.05);观察组治疗前、治疗后24、48 h的FEV1/FVC、FEV1和CC16水平均低于对照组(P<0.05)。治愈、好转和有效患者对应指标均优于无效患者(P<0.05)。观察组FEV1/FVC、FEV1及CC16与临床疗效呈正相关,APACHEⅡ和CPIS评分与临床疗效呈负相关(P<0.05)。结论机械通气对于肺炎患者具有一定的疗效,血清CC16水平与预后相关。Objective To analyze the correlation of serum Clara cell secreted protein 16(CC16)of pneumonia patients and prognosis of mechanical ventilation.Methods A total of 48 pneumonia patients hospitalized and received mechanical ventilation treatment in our hospital were selected as observation group,and 50 healthy person received physical examination at the same period were selected as control group.The clinical indexes of the two groups were compared,correlation between clinical indexes and efficacy of the observation group were analyzed.Results The total effective rate of treatment in the observation group was 83.33%.At 72 h after treatment,the FEV1/FVC,FEV1,level of CC16,scores of APACHEⅡand CPIS in the observation group were better than those before treatment and at 24 h after treatment(P<0.05);FEV1/FVC,FEV1 and CC16 level in the observation group before treatment and at 24,48 h after treatment were lower than those in the control group(P<0.05).The corresponding indexes of cure,improvement and effective patients were better than those of ineffective patients(P<0.05).In the observation group,FEV1/FVC,FEV1 and CC16 were positively correlated with clinical efficacy,scores of APACHEⅡand CPIS were negatively correlated with clinical efficacy(P<0.05).Conclusion Mechanical ventilation on pneumonia patients has certain curative effect,and serum CC16 level is related to the prognosis.
