免气腹腹腔镜手术治疗精索静脉曲张的临床效果

摘　　要：目的探讨免气腹腹腔镜手术治疗精索静脉曲张的临床效果。方法选取2010年9月至2019年8月收治的60例精索静脉曲张患者作为研究对象,35例患者给予免气腹三孔腹腔镜手术,25例患者给予免气腹单孔腹腔镜手术。分析患者的治疗效果。结果患者均成功完成手术,无中转开腹,无气腹辅助手术,无出血等并发症发生,术后当天或次日下床活动、进饮食,术后3~5 d出院。出院后随访3个月,患者无复发,复查精子数量、精子活动率、精子活动度均有明显改善(P<0.05)。采用经脐入路免气腹单孔腹腔镜手术患者术后腹壁无可见瘢痕,有很好的腹壁美容效果。结论免气腹三孔及单孔腹腔镜手术均有一定难度,但确有微创伤、痛苦轻、恢复快、腹壁有很好的美容效果。免气腹腹腔镜手术可减少CO2气体对机体的影响,同时更适合心肺功能不全的患者。Objective To explore the clinical effect of gasless laparoscopic surgery in the treatment of varicocele.Methods From September 2010 to August 2019,60 patients with varicocele were selected as the study objects,35 patients were given gasless three hole laparoscopic surgery,25 patients were given gasless single hole laparoscopic surgery.The treatment effects of the patients were analyzed.Results All the patients successfully completed the operation,no conversion to laparotomy,no pneumoperitoneum assisted operation,no bleeding and other complications occurred.The patients got out of bed and entered the diet on the same day or the next day after the operation,and discharged 3-5 days after the operation.After 3 months follow-up,there was no recurrence,the number of sperm,sperm activity rate and sperm activity were significantly improved,and the differences were statistically significant(P<0.05).There was no visible scar on the abdominal wall of the patients with gasless single hole laparoscopic surgery via umbilical approach,which had a good cosmetic effect on the abdominal wall.Conclusion There are some difficulties in gasless laparoscopic surgery with three or single holes,but there are micro trauma,light pain,fast recovery and good cosmetic effect on abdominal wall.Gasless laparoscopic surgery can reduce the impact of CO2 gas on the body,and is more suitable for patients with cardiopulmonary insufficiency.
