Effect of Wiltse approach for minimally invasive transforaminal lumbar interbody fusion in the treatment of single segment degenerative lumbar spondylolisthesis

摘　　要：目的探讨Wiltse入路微创经椎间孔腰椎椎体间融合术治疗单节段退变性腰椎滑脱的效果。方法选取我院单节段退变性腰椎滑脱患者117例,按手术方案将其分为试验组(n=59)和参照组(n=58)。参照组予以正中入路微创经椎间孔腰椎椎体间融合术,试验组予以Wiltse入路微创经椎间孔腰椎椎体间融合术。比较两组临床效果。结果试验组手术时间、下地活动时间短于参照组,术中失血量、术后引流量少于参照组(P<0.05);术后1个月,两组滑脱角、椎体滑移度均降低(P<0.05),但组间比较,差异无统计学意义(P>0.05);术后6个月,两组JOA评分均升高,且试验组高于参照组(P<0.05);两组并发症总发生率比较,差异无统计学意义(P>0.05)。结论Wiltse入路微创经椎间孔腰椎椎体间融合术治疗单节段退变性腰椎滑脱效果确切,能有效改善滑脱症状及腰椎功能,且并发症少,安全性高。Objective To investigate the effect of Wiltse approach for minimally invasive transforaminal lumbar interbody fusion in the treatment of single segment degenerative lumbar spondylolisthesis.Methods A total of 117 patients with single segment degenerative lumbar spondylolisthesis in our hospital were selected,and they were divided into experimental group(n=59)and reference group(n=58)by the surgical plan.The reference group was treated with median approach for minimally invasive transforaminal lumbar interbody fusion,the experimental group was treated with Wiltse approach for minimally invasive transforaminal lumbar interbody fusion.The clinical effects of the two groups were compared.Results The operation time and activity time of the experimental group were shorter than those of the reference group,the intraoperative blood loss and postoperative drainage were less than those of the reference group(P<0.05).At 1 month after operation,the detachment angle and vertebral slip decreased in the two groups(P<0.05),but there were no significant differences between the two groups(P>0.05).At 6 months after operation,the JOA scores of the two groups increased,and that in the experimental group was higher than the reference group(P<0.05).There was no statistical significance in the total incidence of complications between the two groups(P>0.05).Conclusion Wiltse approach for minimally invasive transforaminal lumbar interbody fusion is effective in the treatment of single segment degenerative lumbar spondylolisthesis,it can effectively improve the spondylolisthesis symptoms and lumbar function,with less complications and high safety.
