摘 要:目的探讨脑电双频指数(BIS)在心脏手术体外循环麻醉监测中的效果。方法选取我院2017年2月至2019年5月收治的146例需进行手术治疗的心脏病患者作为研究对象,随机将其分为观察组和对照组,各73例。两组均行体外循环麻醉下心脏手术,对照组持续静脉输注异丙酚,观察组采用TCI-I输注泵输注异丙酚,通过控制BIS值调整异丙酚用药速度。比较两组患者不同时间点的MAP、CVP、HR、重症停留时间及术后拔管时间。结果T1、T3时,观察组的MAP高于对照组(P<0.05);T2、T4时,观察组的HR高于对照组(P<0.05);两组各个时间点的CVP比较,差异均无统计学意义(P>0.05)。观察组的重症停留时间及术后拔管时间均短于对照组(P<0.05)。结论BIS应用于心脏手术体外循环麻醉中麻醉深度的监测,可维持患者的血流动力学稳定,缩短患者的重症停留时间及术后拔管时间。Objective To investigate the effect of bispectral index(BIS)in the monitoring of cardiopulmonary bypass anesthesia in cardiac surgery.Methods From February 2017 to May 2019,146 patients with heart disease who needed surgical treatment in our hospital were selected as the study objects,and randomly divided into observation group and control group,with 73 cases in each group.Cardiac surgery under cardiopulmonary bypass anesthesia was performed in both groups.The control group received continuous intravenous infusion of propofol,the observation group used a TCI-I infusion pump to infuse propofol,and the propofol administration speed was adjusted according to BIS value.The levels of MAP,CVP,HR at different time points,critical stay time and postoperative extubation time were compared between the two groups.Results At T1 and T3,the MAP in the observation group were higher than those in the control group(P<0.05);at T2 and T4,the HR in the observation group were higher than those in the control group(P<0.05);there were no significant differences in CVP at different time points between the two groups(P>0.05).The time of critical stay and postoperative extubation in the observation group were shorter than those in the control group(P<0.05).Conclusion BIS is used to monitor the depth of anesthesia during cardiopulmonary bypass anesthesia in cardiac surgery,which can maintain hemodynamic stability,shorten the patients'critical stay time and postoperative extubation time.
