原发干燥综合征合并肾小管间质病变中异位生发中心的应用及其意义分析

摘　　要：目的探讨原发干燥综合征合并肾小管间质病变中异位生发中心的应用情况。方法选取2018年2月至2019年2月本院收治的原发干燥综合征合并肾小管间质病变患者40例作为研究组,同时选取单纯的原发干燥综合征患者40例作为对照组。收集患者一般资料,对相关指标进行检测。比较两组患者肾脏情况、实验室检查结果。结果研究组肾小球硬化指数为(0.5±0.2),血肌酐为(143.4±67.1)mmol/L,24 h尿蛋白定量为(0.7±0.3)g/24 h,肾小球滤过率为(65.9±4.4)ml/min;对照组肾小球硬化指数为(0.6±0.3),血肌酐为(250.7±112.5)mmol/L,24 h尿蛋白定量为(1.3±0.5)g/24 h,肾小球滤过率为(40.1±17.6)ml/min;研究组肾小球硬化指数、血肌酐、24 h尿蛋白定量小于对照组(P<0.05),研究组肾小球滤过率高于对照组(P<0.05)。研究组血磷、血钾、血气HCO3、血气PH5、血红蛋白水平及血γ球蛋白升高、低钙血症、低白蛋白血症、贫血发生率低于对照组(P<0.05),研究组血钙、血清白蛋白水平及低磷血症、低钾血症、代谢性酸中毒发生率高于对照组(P<0.05)。结论原发干燥综合征合并肾小管间质病变中异位生发中心的应用及其意义分析中,发现异位生发中心在患者的肾小管间质病变中发挥着重要的作用。Objective To investigate the application of ectopic germinal center in primary Sjogren's syndrome with renal tubulointerstitial lesions.Methods Forty patients with primary Sjogren's syndrome with renal tubulointerstitial lesions admitted to our hospital from February 2018 to February 2019 were selected as the study group,and 40 patients with primary Sjogren's syndrome were selected as the control group.The general data of patients was collected and related indicators were detected.The kidney condition and laboratory test results of the two groups were compared.Results The glomerulosclerosis index of the study group was(0.5±0.2),the serum creatinine was(143.4±67.1)mmol/L,the 24 h urine protein was(0.7±0.3)g/24 h,and the glomerular filtration rate was(65.9±4.4)ml/min,those of the control group were(0.6±0.3),(250.7±112.5)mmol/L,(1.3±0.5)g/24 h,(40.1±17.6)ml/min;the glomerulosclerosis index,serum creatinine,24 h urine protein of the study group were lower than those of the control group(P<0.05),the glomerular filtration rate of the study group was higher than that of the control group(P<0.05).The levels of blood phosphorus,blood potassium,blood gas HCO3,blood gas PH5,hemoglobin,and the incidences of increased blood gamma globulin,hypocalcemia,hypoalbuminemia,anemia of the study group were lower than those of the control group(P<0.05),and the levels of blood calcium,serum albumin,and the incidences of hypophosphatemia,hypokalemia,metabolic acidosis of the study group were higher than those of the control group(P<0.05).Conclusion The application of ectopic germinal center in primary Sjogren's syndrome with renal tubulointerstitial lesions and its significance analysis show that ectopic germinal center plays an important role in renal tubulointerstitial lesions.
