摘　　要：目的了解三级甲等医院住院患者临床决策和医患关系的现状,探索新的社会矛盾下医疗决策模式对医患关系的影响。方法采用横断面调查,对中南大学湘雅二医院10个护理单元(内科片和外科片各5个)符合纳入标准的住院患者发放一般资料调查表、患者决策参与期望量表、患者对医疗决策参与满意度调查问卷、医患关系问卷,采用SPSS 22.0对以上资料进行统计学处理。结果共收回有效问卷256份,其中139名(54.30%)患者选择医患共同决策模式,67名(26.17%)患者选择主动决策模式,50例(19.53%)患者选择被动决策模式。选择医患共同决策模式患者对医疗服务的总满意度得分为(76.73±2.32)分,明显高于主动与被动角色模式患者对医疗服务的满意度得分,差异具有统计学意义(P<0.05)。结论医患共同决策模式有助于提高患者满意度,促进和谐医患关系。【Objective】To explore the current situation and influence factors of clinical decision-making and doctor-patient relationship of inpatients under the new social contradictions.【Methods】A total of 266 patients were randomly selected from our hospital,and they were asked to finish questionnaires including the Control Preference Scale(CPS),satisfaction survey questionnaire and doctor-patient relationship questionnaire.SPSS 22.0 was used for statistical analysis.【Results】Totally 256 patients completed the survey,in whom 139 patients(54.30%)preferred to shared-decision making mode,67 patients(26.17%)selected active-decision making mode,50 patients(19.53%)selected passive-decision making mode.The total satisfaction score of medical services of patients selecting shared-decision making mode was(76.73±2.32),which was significantly higher than that of the other two groups,and the total satisfaction score among each groups was statistically different(P<0.05).【Conclusion】The shared-decision making mode is helpful to improve patient satisfaction and promote harmonious relationship between doctors and patients.
