腔镜深筋膜下交通支离断术对下肢静脉曲张性溃疡患者的临床效果

摘　　要：目的探索腔镜深筋膜下交通支离断术对下肢静脉曲张性溃疡患者的临床效果。方法选取102例下肢静脉曲张性溃疡患者为研究对象,根据手术方法分为观察组和对照组,各51例。对照组采用传统大隐静脉高位结扎、剥脱术治疗,观察组在传统高位结扎、剥脱基础上再进行腔镜深筋膜下交通支离断术治疗。观察两组患者3个月内复发率、患肢周径增长长度、患肢皮肤温度、血流速度及静脉血流峰速度。结果观察组3个月内复发率低于对照组(P<0.05)。观察组患肢周径增长长度短于对照组(P<0.05)。治疗后,观察组患肢皮肤温度、血流速度、静脉血流峰速度高于对照组(P<0.05)。结论对下肢静脉曲张性溃疡患者在传统高位结扎、剥脱术基础上实施腔镜深筋膜下交通支离断术治疗效果显著。Objective To explore the clinical effect of endoscopic deep fascia traffic branch distraction on patients with varicose ulcer of lower extremity.Methods A total of 102 patients with varicose ulcer of lower extremity were selected as the study objects.According to the surgical method,the patients were divided into observation group and control group,with 51 cases in each group.The control group was treated with traditional high ligation and stripping of great saphenous vein,and the observation group was treated with endoscopic deep fascial traffic branch distraction on the basis of traditional high ligation and stripping.The recurrence rate within 3 months,peripheral diameter growth length,skin temperature,blood flow velocity and peak velocity of venous blood flow were observed between the two groups.Results The recurrence rate in the observation group was lower than that in the control group within 3 months(P<0.05).The growth length of peripheral diameter in the observation group was shorter than that in the control group(P<0.05).After treatment,the skin temperature,blood flow velocity and peak velocity of venous blood flow in the observation group were higher than those in the control group(P<0.05).Conclusion It is effective to treat varicose ulcer of lower extremity by endoscopic deep fascial traffic branch distraction on the basis of traditional high ligation and exfoliation.
