摘　　要：目的观察胃大部分切除术后患者胃肠功能情况,评价以蒲元和胃胶囊联合艾司奥美拉唑的中西医结合治疗方法促进胃大部分切除术后患者胃肠功能恢复的临床疗效。方法将2015年5月~2016年7月在我院行胃大部分(近端/远端)切除术的患者随机分为两组,以术后第十天(开始进食时间)作为起始服药时间,对照组采取单用艾司奥美拉唑(耐信)口服治疗,实验组在对照组基础上加用蒲元和胃胶囊。开始服药后每周进行电话随访,随访时间为四周。结果两组患者症状积分在起始服药阶段无显著差异,差异无统计学意义(P>0.05),在随访期内均呈现出先升高后下降的趋势,远端胃切除患者各项症状积分均显著低于近端胃切除患者,实验组各项症状积分均不同程度低于对照组。结论胃大部分切除术后胃肠功能不良是术后常见并发症,且其严重程度近端胃切除显著高于远端胃切除,蒲元和胃胶囊联合艾司奥美拉唑能显著改善胃大部分切除术后患者功能性消化不良的临床症状,促进患者胃肠功能恢复,具有较好的临床疗效。Objective We observed the situation of gastrointestinal function in patients underwent partial gastrectomy,to evaluate the clinical efficacy of combined therapy of Chinese and Western medicine,which combines Puyuan Hewei capsules combined with esomeprazole,on the recovery of gastrointestinal function in postoperative patients who underwent partial gastrectomy.Methods One hundred and ninety nine patients who underwent partial gastrectomy, include proximal gastrectomy and distal gastrectomy, in our hospital from May 2015 to July 2016, were randomly divided into a study group and a control group.102 patients in the study group and 97 patients in the control group.The study group was given Puyuan Hewei capsules combined with esomeprazole,while the control group was only given esomeprazole.Ten days after the operation, participants started taking the medicine.The follow-up time is 4 weeks, telephone follow-up was carried out every weeek.Results The symptomatic scores had no significant difference between the study group and control group (P>0.05) when paticipants started taking the medicine.The trends of the symptomatic scores in both the study group and control group were raise first and then declined.The symptomatic scores in patients who underwent distal gastrectomy were significantly lower than the ones who underwent proximal gastrectomy.The symptomatic scores in the study group were lower than the control group.Conclusion Gastrointestinal dysfunction after partial gastrectomy is a common postoperative complication.Moreover,the symptoms of gastrointestinal dysfunction after proximal gastrectomy is much more severe than distal gastrectomy. Puyuan Hewei capsules combined with esomeprazole could markedly improve the symptoms of gastrointestinal dysfunction after partial gastrectomy compare with monotherapy with esomeprazole. And the situation of gastrointestinal function in patients could also be significantly improved by the combination of Puyuan Hewei capsules and esomeprazole.
