摘 要:目的不同全麻药物对电子耳蜗植入术(CI)婴幼儿肠道菌群多样性的影响尚不明确。文中主要观察丙泊酚和七氟烷对CI婴幼儿肠道菌群多样性的影响。方法选取2018年1月至8月在上海交通大学医学院附属第九人民医院麻醉科接受CI手术的20例听障患儿。随机分为丙泊酚组(静脉全麻药物丙泊酚)和七氟烷组(吸入全麻药物七氟烷),每组10例。提取基因组DNA进行PCR技术,并用16S rDNA测序技术进行组间菌群多样性(肠道菌群分类学组成、菌群alpha多样性指数、肠道菌群PICRUSt功能预测)分析。结果肠道菌群分类学组成分析显示,术后2组婴幼儿肠道菌群均以厚壁杆菌门和拟杆菌门为主要菌门,2组对肠道菌群各菌门种类(放线菌门、拟杆菌门、厚壁杆菌门等)数量的影响差异无统计学意义(P>0.05)。菌群alpha多样性指数分析显示,2组各度量指标(物种信息指数、chao1指数、香浓指数等)结果差异无统计学意义(P>0.05)。肠道菌群PICRUSt功能预测分析显示,七氟烷组在KO功能中蛋白激酶代谢(0.32±0.03)、肽酶代谢(1.82±0.08)、结核病通路(0.14±0.01)与丙泊酚组(0.28±0.03、1.91±0.07、0.15±0.01)比较差异有统计学意义(P<0.05)。结论七氟烷和丙泊酚通过影响肠道菌群功能多样性中KO功能的不同代谢通路,进而调节肠道菌群功能多样性的变化,为CI婴幼儿的临床麻醉药物使用提供了新的指导。Objective The correlation of intestinal flora diversity in infants undergoing cochlear implantation(CI)with different general anesthetics remains unclear.The aim of this study was to observe the effects of propofol and sevoflurane on intestinal flora diversity in infants undergoing cochlear implantation.Methods From January 2018 to August 2018,twenty infants with hearing impairment who underwent CI in the Department of Anesthesiology,Ninth People's Hospital,Shanghai Jiaotong University School of Medicine were randomly divided into propofol group(10 cases)and sevoflurane group(10 cases).The propofol group(group P)received intravenous injection while the sevoflurane group(group S)received inhalation.Genomic DNA was extracted for PCR and 16S rDNA sequencing technique was used to analyze the diversity of intestinal flora(intestinal flora taxonomic composition,flora alpha diversity index,and intestinal flora PICRUSt function prediction).Results The analysis of intestinal flora taxonomic composition showed that the intestinal flora of infants in the two groups were mainly Firmicutes and Bacteroidetes after operation.There was no significant difference in species diversity of intestinal flora(Actinobacteria,Bacteriodetes,Firmicutes,etc.)between Group S and Group P(P>0.05).The analysis of flora alpha diversity index showed that there was no significant difference in metrics(species information index,chaol index,Shannon index,etc.)between Group S and Group P(P>0.05).The analysis of intestinal flora PICRUSt function prediction showed that there were significant differences in Peptidase metabolism(1.82±0.08,1.91±0.07,P=0.02),Protein Kinase metabolism(0.32±0.03,0.28±0.03,P=0.02)and Tuberculosis pathway(0.14±0.01,0.15±0.01,P=0.049)between Group S and Group P after operation.Conclusion Sevoflurane and propofol can regulate the functional diversity of intestinal flora through affecting different metabolic pathways of KO function in the functional diversity of intestinal flora and provide new guidance for the use of clinic
