摘　　要：目的探讨胃液培养在新生儿感染性肺炎辅助诊断中的价值。方法选择2016—2017年我院新生儿重症监护病房收治的新生儿,将281例感染性肺炎新生儿设为病例组,559例非感染性疾病新生儿设为对照组,收集两组患儿的胃液培养检查结果,并分析其与新生儿感染高危因素的关系。结果病例组胃液培养阳性率为39.86%(112/281),高于对照组的31.48%(176/559),差异有统计学意义(χ^2=5.82,P<0.05)。新生儿胃液细菌培养阳性患儿中羊水粪染、胎膜早破、脐带绕颈情况更为多见(χ^2=10.01、7.63和10.63,P<0.05)。结论新生儿胃液细菌培养对新生儿感染性肺炎的诊断及抗感染治疗或有参考价值。Objective To explore the value of gastric juice culture in the assistant diagnosis of neonates with infectious pneumonia.Methods Preterm infants were collected in the neonatal intensive care unit(NICU)from 2016 to 2017.They were divided into case group(n=281)and control group(n=559).The results of gastric culture examination were analyzed and the positive cases of gastric culture examination were analyzed.Results The positive rate of gastric culture examination in the case group was 39.86%,which was significantly higher than that in the control group(31.48%,χ2=5.82,P<0.05).The neonatal gastric juice culture was mainly Gram-negative bacilli,followed by Gram-positive cocci.More infants with bacterial culture positive had the meconium stained amniotic fluid,premature rupture of membranes,umbilical cord around neck before they were born(χ2=10.01,7.63 and 10.63,P<0.05).Conclusions The positive bacterial culture of neonatal gastric juice is associated with neonatal infectious pneumonia,which may have great significance for the diagnosis and anti-infective treatment of neonatal infectious pneumonia.
