摘 要:目的观察右美托咪定复合硝酸甘油行快速麻醉诱导对血流动力学影响和麻醉效果。方法 22名择期全麻手术患者随机分为D组和DN组两组。患者入室后右美托咪定慢诱导组(D组,n=11)用0.0004%右美咪定按0.8 μg/kg的剂量,以100 ml/h速度静脉输注,右美托咪定复合硝酸甘油快诱导组DN组(n=11)用0.0004%右美咪定按0.8 μg/kg的剂量复合0.0025%硝酸甘油按5 μg/kg的剂量,以1000 ml/h的速度静脉输注,统计从给药开始(T1),给药开始3 min(T2),给药开始5 min(T3)至给药开始10 min(T4)时,两组的SBP,DBP,MAP,HR以及从给药开始至Ramsay镇静评分达到5分所需要的时间(Time)。结果两组各时间点的血压未见明显差异,DN组的T2和T4时的心率为72.43±4.86次/分和68.71±4.19次/分,显著高于D组58.86±2.48次/分和54.00±1.41次/分。DN组Time 3.71±0.49 min与D组Time6.43±2.07 min相比显著缩短(P<0.05)。结论右美托咪定复合硝酸甘油进行快速麻醉诱导与单纯右美托咪定慢诱导相比,患者血流动力学更加平稳,能更快达到预期的镇静深度。Objective To observe the effect on hemodynamics and anesthesia depth induced by rapid intravenous infusion with dexmedetomidine combined with nitroglycerin.Methods Group D(n=10)with 0.0004%by 0.8μg/kg dose of intravenous infusion at 100 ml/h speed,Group DN(n=10)with 0.0004%set at 0.0025%of the 0.8μg/kg dose combined with nitroglycerin by 5μg/kg,intravenous infusion at a speed of 1000 ml/h,recorded from the start(T1),after 3 min(T2),5 min(T3)and 10 min(T4),the SBP,DBP,MAP,HR and the Time it takes from the start of administration until the Ramsay sedation score reaches 5 of two groups.Results There was no significant difference in blood pressure between the two groups at each time point.The heart rate at T2 and T4 of group DN was 72.43±4.86 bpm and 68.71±4.19 bpm,which were significantly higher than that of group D at 58.86±2.48 bpm and 54.00±1.41bpm.3.71±0.49s in Time of DN group was significantly shortened compared with 6.43±2.07s in Time of D group(P<0.05).Conclusion Dexmedetomidine combined with nitroglycerin showed more stable hemodynamics and faster sedation than that intravenous administration with dexmedetomidine alone.
