尿激酶与地塞米松联合治疗早期粘连性结核性胸膜炎的效果观察

摘　　要：目的探讨尿激酶与地塞米松联合治疗早期粘连性结核性胸膜炎的效果。方法选取本院2015年5月至2017年5月收治的早期粘连性结核性胸膜炎患者200例作为研究对象,随机分为对照组和治疗组,各100例。对照组给予常规治疗方案,治疗组在常规方案的基础上采用尿激酶与地塞米松联合治疗方案。观察两组患者的胸腔积液消失的时间、胸膜的厚度以及肺功能状况。结果治疗组的胸腔积液消失时间(14.3±2.5)min少于对照组(18.7±3.1)min,组间比较差异具有统计学意义(P<0.05),治疗组的胸膜厚度(1.07±0.22)mm小于对照组(1.49±0.26)mm,组间比较差异具有统计学意义(P<0.05)。治疗组患者第一秒用力呼气量(89.85±8.32)多于对照组(83.24±5.46),组间比较差异具有统计学意义(P<0.05)。对照组患者总有效率为93%低于治疗组的99%,组间比较差异具有统计学意义(P<0.05)。治疗过程中两组患者无显著的不良反应,差异无统计学意义。结论早期粘连性结核性胸膜炎患者使用尿激酶与地塞米松联合治疗具有显著的临床治疗效果,值得临床推广应用。Objective To investigate the effect of urokinase combined with dexamethasone in the treatment of early adhesive tuberculous pleurisy.Methods A total of 200 patients with early adhesive tuberculous pleurisy admitted to our hospital from May 2015 to May 2017 were enrolled in this study.They were randomly divided into control group and treatment group,with 100 cases in each group.The control group was given a conventional treatment regimen,and the treatment group was treated with a combination of urokinase and dexamethasone on the basis of a conventional regimen.The time to disappear of pleural effusion,the thickness of the pleura,and the lung function were observed in the two groups.Results The disappearance time of pleural effusion in the treatment group(14.3±2.5)min was less than that in the control group(18.7±3.1)min,and the difference between the groups was statistically significant(P<0.05).The pleural thickness of the treatment group(1.07±0.22)mm was significantly lower than that of the control group(1.49±0.26)mm,and the difference was statistically significant(P<0.05).In the treatment group,the forced expiratory volume in the first second(89.85±8.32)was significantly higher than that in the control group(83.24±5.46),which the difference was statistically significant(P<0.05).The total effective rate was 93%in the control group lower than the total effective rate in the treatment group 99%.The difference was statistically significant(P<0.05).There were no significant adverse reactions in the two groups of patients during the treatment,and there was no statistical significance.Conclusion The combination of urokinase and dexamethasone in patients with early stage tuberculous pleurisy has significant clinical effects and is worthy of clinical promotion and application.
