泮托拉唑与蒲元和胃及百乐眠胶囊联用致过敏性紫癜的分析

摘　　要：目的分析泮托拉唑与蒲元和胃及百乐眠胶囊联用后出现的不良反应,为临床安全用药提供参考。方法选择青岛大学附属医院皮肤科2019年3月收治的1例联用泮托拉唑与蒲元和胃及百乐眠胶囊后出现过敏性紫癜的患者,分析不良反应的发生发展过程及临床特点。结果患者主诉双下肢瘀点、瘀斑5 d。5 d前因“腹痛”给予口服泮托拉唑钠肠溶胶囊、蒲元和胃胶囊和百乐眠胶囊。用药1 h后双小腿出现红色瘀点、瘀斑,诊断为过敏性紫癜。抗过敏治疗3周后患者皮疹完全消退。结论泮托拉唑致过敏性紫癜可能是PPI的“类反应”,但不能排除蒲元和胃胶囊和百乐眠胶囊致过敏性紫癜的可能,中西药多种药物联用时应谨慎。Objective To analyze the adverse reactions(ADRs)induced by combination of pantoprazole,Puyuan Hewei and Bailemian Capsules,providing references for clinical safety and rational use of drugs.Methods A case admitted in Department of Dermatology,the Affiliated Hospital of Qingdao University at March 2019 was study who appeard Henoch-Schonlein Purpura after using a combination of pantoprazole,Puyuan Hewei and Bailemian Capsules to analyze the clinical manifestations of ADRs.Results The patient'chief complaints were ecchymosis and petechia at duoble lambs for 5 days.5 days ago,the patient oral a combination of pantoprazole,Puyuan Hewei and Bailemian Capsules because of abdominal pain.After 1 hour of drugs administration,duoble lambs appeared ecchymosis and petechia.Then the patient diagnosed with Henoch-Schonlein Purpura.The lesions completely subsided after 3 weeks of anti-allergic treatment.Conclusion Henoch-Schonlein Purpura induced by pantoprazole may be'class effect'of PPIs due to their substituted benzimidazole structure,but we cannot rule out the possibility that the adverse drug reaction induce by Puyuan Hewei or Bailemian Capsules.We should be more careful when using combination of modem and traditional medicines.
