摘　　要：目的探讨持续封闭负压引流(VSD)联合重组人碱性成纤维细胞生长因子(rh-bFGF)外用溶液冲洗治疗难愈性创面的治疗效果。方法选取2018年1月-2019年12月我院收治的难愈性创面修复患者40例,按照随机数字表法将其分为试验组和对照组,各20例。两组患者创面进行彻底清创处理后,给予VSD进行创面封闭,其中试验组予加入rh-bFGF的生理盐水持续冲洗进行治疗;对照组以同样方法给予等量生理盐水持续冲洗,观察两组肉芽成熟度、治疗后创面面积、换药次数、总住院时间。结果试验组在治疗期间,换药次数、创面愈合时间和住院时间均显著少于对照组,第7天、第14天试验组肉芽成熟率分别为60%、75%,显著优于对照组(10%、35%),具有统计学意义(P<0.05)。结论对于难愈性创面患者,在VSD基础上联合rh-bFGF可显著提高治疗效果,促进肉芽的形成,缩短创面愈合时间,值得临床推广应用。Objective To investigate the clinical effect of vacuum sealing drainage(VSD)combined with recombinant human basic fibroblast growth factor(rh-bFGF)external solution in the treatment of refractory wound.Methods A total of 40 cases with refractory wound admitted to our hospital from January 2018 to December 2019 were recruited and randomly divided into experimental group and control group in accordance with random number table,with 20 cases in each group.After thorough debridement,VSD was given to seal the wound,then the experimental group was treated by continuous irrigation with rh-bFGF normal saline,while the control group was given the same amount of normal saline for continuous irrigation in the same way.Comparisons were conducted between the two groups on granulation tissue maturity,wound area,frequency of dressing change and total length of hospital stay.Results During the treatment period,the frequency of dressing change,wound healing time and total length of hospital stay in experimental group were significantly lower than those in the control group.Besides,The granulation maturation rates in experimental group on day 7 and day 14 were 60%and 75%respectively,which were significantly better than the control group(10%and 35%),with statistically significant differences(P﹤0.05).Conclusion For patients with chronic refractory wound,the comprehensive treatment with VSD and rh-bFGF could significantly improve the therapeutic effect,accelerate the formation of granulation,and shorten the healing time of wound,which is worthy for clinical promotion and application.
