摘 要:目的探讨体检人群糖化血红蛋白(HbA1c)分层研究与血糖血脂的关系,分析血糖、糖化血红蛋白和血脂的变化情况,了解血糖的波动状况。方法选取622例来院体检人员,其中男336名,女286名,平均(53.91±14.18)岁。分别采用已糖激酶法检测空腹血糖(FPG),高效液相色谱法检测HbA1C,酶法检测甘油三酯(TG),总胆固醇(TC),高密度脂蛋白胆固醇(HDL)。将HbA1C按不同水平分成5层,A<5.7%,5.7%≤B<6.5%,6.5%≤C<7.0%,7.0%≤D<9.0%,E≥9.0%。分别比较HbA1C不同分层时,FPG,TG,TC,HDL检测值的变化情况。结果随着HbA1C分层水平的升高,FPG明显升高(P<0.01),TG、TC在B、C组的变化中无明显的统计学差异(P>0.05),在D、E组的变化中TG水平明显升高(P<0.01),TC的变化不太明显,仅在E组中有明显的升高(P<0.01);HDL的变化在B、C、D组中有统计学差异(P<0.01),在E组的变化中无明显的统计学差异(P>0.05);在D、E二组间的比较中,仅HbA1C和FPG变化明显(P<0.01),TG、TC、HDL变化均不明显。FPG与HbA1C呈明显的正相关(r=0.294 3,0.437 7,0.532 0,0.615 8),TG仅在B、D组呈明显的正相关(r=0.199 4,0.231 7),TC在A、D、E组呈正相关(r=0.206 7,0.140 3,0.203 8),HDL和HbA1C在B、C、D组呈负相关(r=-0.147 8,-0.366 6,-0.119 3)。结论 HbA1C按不同水平分层后,可以更加清晰地了解HbA1C与空腹血糖及血脂在不同浓度水平的变化关系,这对患者自身状况的了解和血糖波动变化的掌握有较重要的作用,在糖尿病的预防、诊断和治疗上会产生明显的积极的效果。Objective To explore the relationship between HbA1 c stratification and blood glucose and lipids, analyze the changes of blood glucose, HbA1 c and lipids, and understand the fluctuation of blood glucose. Methods 622 physical examiners were selected, including 336 males and 286 females, with an average age of(53.91 ± 14.18). Fasting blood glucose(FPG) was detected by hexokinase, HbA1 c by HPLC, triglyceride(TG), total cholesterol(TC) and high density lipoprotein cholesterol(HDL) by enzyme. HbA1 c was divided into 5 layers according to different levels, a<5.7%, 5.7%<B<6.5%, 6.5%<C<7.0%, 7.0%<d<9.0%, e ≥ 9.0%. The changes of FPG, TG, TC and HDL in different layers of HbA1 c were compared. Results With the increase of HbA1 c stratification level, FPG increased significantly(P<0.01), TG and TC had no significant difference in the changes of group B and C(P >0.05), TG level increased significantly in the changes of group D and E(P<0.01), TC change was not obvious, only in Group E(P<0.01), HDL change had significant difference in the changes of group B, C and D(P<0.01), There were no statistical significantly differences in the change of group E(P >0.05). In the comparison between groups D and E, only HbA1 c and FPG changed significantly(P<0.01), TG, TC and HDL did not change significantly. FPG was positively correlated with HbA1 c(r=0.2943, 0.4377, 0.5320, 0.6158), TG was positively correlated with group B and D(r=0.1994, 0.2317), TC was positively correlated with group A, D and E(r=0.2067, 0.1403, 0.2038), HDL and HbA1 c were negatively correlated with group B, C and D(r=-0.1478,-0.3666,-0.1193). Conclusion After HbA1 c is stratified according to different levels, the relationship between HbA1 c and fasting blood glucose and blood lipid in different concentration levels can be more clearly understood, which plays an important role in the understanding of patients’ own conditions and mastering the changes of blood glucose fluctuation, and will produce significant positive effects in the prevention, diagnosis and t
