不同冲管液在预防鼻肠管肠内营养堵管中的效果观察

摘　　要：目的探讨不同冲管液在预防鼻肠管肠内营养堵管中的效果。方法收集2017年6月至2018年12月南京医科大学附属南京医院普外科120例接受鼻肠管肠内营养的胃癌术后患者,将患者按随机数字表分为对照组和试验组,每组各60例。对照组采用常规温开水30 ml脉冲式冲管方法,试验组使用胰酶混合5%碳酸氢钠30 ml脉冲式冲管方法。观察比较两组患者鼻肠管肠内营养的堵管率及堵管发生时间。结果试验组堵管率为1.7%(1/60),明显低于对照组的13.3%(8/60)(P<0.05);试验组鼻肠管肠内营养堵管发生时间为置管后第16天,晚于对照组置管后的(6.2±1.4)天。结论胰酶混合碳酸氢钠溶液冲管能有效推迟鼻肠管堵管发生时间,降低鼻肠管肠内营养的堵管率。Objective To investigate the effect of different flushing fluids on preventing the obstruction of nasojejunal tubes in patients with enteral nutrition.Methods One hundred and twenty postoperative patients with gastric cancer receiving enteral nutrition from the General Surgery Department of Nanjing Hospital Affiliated to Nanjing Medical University during June 2017 to December 2018 were divided into control group and experimental group randomly,with 60 patients in each group.The patients in control group were treated by the method of convention alim pulse type with warm water.And the patients in experimental group were treated with creon delayed-release pancreatic enzyme mixed with sodium bicarbonate.The plugging rate and the time of blockage in nasojejunal tubes between the two groups were compared.Results The plugging rate in experimental group was significantly lower than that in the control group(P<0.05).And the time of blockage in nasojejunal tube was longer than that in the control group(P<0.05).Conclusions The use of pancreatic enzyme mixed with sodium bicarbonate solution can effectively shorten the plugging time of the nasojejunal tube and reduce the incidence of blockage.
