复方血栓通联合激光治疗糖尿病黄斑水肿的疗效及对血清VEGF、PDGF、IL-1水平的影响

摘　　要：目的探讨复方血栓通联合激光治疗糖尿病黄斑水肿(DME)患者的疗效及对血清血管内皮生长因子(VEGF)、血小板源性生长因子(PDGF)、白细胞介素-1(IL-1)水平的影响。方法根据数字表法将河北省武安市第一人民医院2016年12月—2018年12月收治的120例DME患者随机分为对照组(n=60)和研究组(n=60),对照组给予激光治疗,研究组在对照组治疗基础上联合复方血栓通口服,比较2组患者临床疗效,血清VEGF、PDGF、IL-1水平,眼部情况(视力、微血管瘤数、黄斑区荧光面积、眼底出血面积)及2组治疗期间不良反应情况。结果研究组治疗3个月后的临床总有效率为75.0%(45/60),显著高于对照组患者的48.3%(29/60)(P<0.05)。2组患者治疗3个月后血清VEGF、PDGF、IL-1水平均明显下降(P均<0.05),且研究组均明显低于对照组(P均<0.05)。2组患者治疗3个月后视力均升高(P均<0.05),且研究组高于对照组(P<0.05);微血管瘤数、黄斑区荧光面积、眼底出血面积均明显降低(P均<0.05),且研究组低于对照组(P均<0.05)。2组不良反应发生率比较差异无统计学意义(P>0.05)。结论复方血栓通联合激光治疗DME疗效显著,可能与降低血清VEGF、PDGF、IL-1水平有关,且不增加不良反应。Objective It is to investigate the effect of Compound Xueshuantong combined with laser treatment on diabetic macular edema(DME)and its effects on the levels of serum vascular endothelial growth factor(VEGF),platelet-derived growth factor(PDGF),and interleukin-1(IL-1).Methods 120 patients with DME treated in the First People’s Hospital of Wu’an City from December 2016 to December 2018 were randomly divided into a control group(n=60)and a study group(n=60)according to random number method.The control group was given laser treatment.The study group was treated with compound Xueshuantong based on the control group.The clinical efficacy,levels of serum VEGF,PDGF,IL-1,eye status,eye area,and adverse reactions occurred during treatment in the two groups were compared.Results The total clinical effective rate of the study group after 3 months of treatment was 75.00%(45/60),which was significantly higher than that of the control group of 48.33%(29/60)(P<0.05).The levels of serum VEGF,PDGF,and IL-1 was significantly decreased in both groups after 3 months of treatment,and the study group was lower than the control group(P<0.05).The visual acuity was significantly increased after 3 months of treatment in both groups,and the study group was higher than the control group(P<0.05);the number of microhemangiomas,macular area,and fundus hemorrhage area were significantly reduced in the two groups,and the study group was lower than the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Compound Xueshuantong combined with laser treatment has a significant curative effect on DME.It can effectively improve the serum VEGF,PDGF,IL-1 levels,vision,and number of microhemangiomas,reduce the area ofmacular fluorescence and fundus hemorrhage without increasing the incidence of adverse reactions.
