玉肤褪红汤对风热血热型激素依赖性皮炎患者的临床疗效及TEWL、血清IL-4、IFN-γ水平的影响

摘　　要：目的观察玉肤褪红汤治疗急性期风热血热型激素依赖性皮炎患者的临床疗效及其对经皮水分丢失(TEWL)及血清IL-4、IFN-γ水平的影响。方法收集确诊的风热血热型急性期激素依赖性皮炎患者80例,随机分为观察组与对照组各40例,两组均予吡美莫司乳膏外涂及盐酸西替利嗪片口服,观察组加服玉肤褪红汤,治疗4周。比较治疗前后两组症状积分、经皮水分丢失及血清IL-4、IFN-γ改变情况。结果观察组有效率80.00%,对照组有效率65.00%,差异具有统计学意义(P<0.05)。治疗前观察组与对照组TEWL、IL-4、IFN-γ水平比较差异无统计学意义(P均>0.05)。治疗4周后,观察组TEWL和IL-4水平显著降低,IFN-γ水平升高,与治疗前比较差异具有统计学意义(P<0.05);对照组TEWL水平降低,IFN-γ水平升高,与治疗前比较差异具有统计学意义(P<0.05),IL-4水平变化不明显(P>0.05)。结论玉肤褪红汤可以改善风热血热型急性期激素依赖性皮炎患者的临床症状、体征及皮肤屏障功能,抑制皮肤炎症反应,疗效确切。Objective To investigate the clinical effect of Yufu Tuihong decoction and its influence on Transepidermal water loss(TEWL),as well as IL-4 and IFN-γlevels in serum in patients with hormone dependent dermatitis(HDD)of acute wind-heat and blood-heat type.Methods Eighty patients with HDD of acute wind-heat and bloodheat type were collected,and then randomly divided into observation group(n=40)and control group(n=40).In 4 weeks of therapy,both groups were treated with pimecrolimus ointment and oral administration of cetirizine tablets.In addition,the observation group was supplemented with Yufu Tuihong decoction.The symptom scores,TEWL,IL-4 and IFN-γlevels before and after treatment between two groups were compared.Results The effective rate in observation group(80.00%)was significantly higher than that(65.00%)in control group(P<0.05).There were no significant differences in TEWL,as ell as IL-4 and IFN-γlevels in both groups before treatment(P>0.05).After 4 weeks,the TEWL and IL-4 level of observation group were significantly reduced,while IFN-γlevel was significantly increased,compared with before treatment(P<0.05).In control group,TEWL was significantly reduced(P<0.05),the change of IL-4 had no statistically significant difference(P>0.05),and IFN-γwas increased,compared with before treatment(P<0.05).Conclusion Yufu Tuihong decoction can significantly improve the clinical symptoms and signs of acute wind-heat and blood-heat type of HDD patients.It can also improve the function of skin barrier and inhibit inflammatory response,with good therapeutic effects.
