国医大师李济仁治疗痹病用药规律分析

摘　　要：目的:分析国医大师李济仁治疗风湿性关节炎、类风湿关节炎、颈椎病、腰痹病、痛风5种疾病的用药规律与"异病同治"法则在痹病治疗中的临床经验及其学术思想。方法:收集2013年1月至2018年12月李济仁教授诊治5种疾病的处方,运用Excel表与基于网络的中药方剂配伍规律分析方法对纳入数据进行分析。结果:通过分析得到5种疾病的常用中药、常用药对以及中药配伍网络。李济仁教授对5种疾病治疗多使用藤类药、虫类药、补虚药、活血药以及健脾药,药物多入肝经、肾经、脾经,注重寒温药物的平衡。风湿性关节炎和类风湿关节炎常用清络饮方剂,而类风湿关节炎又多用益肾清络活血方。颈椎病和腰痹病的用药极为相似。痛风和其余4种疾病的差异较大。结论:通过分析用药规律,可以发现李教授主要以固本培元、平调寒热、治血通络、祛风止痛为治则,对于不同的疾病分别采用不同的侧重点进行治疗,体现在痹病范围内"异病同治"的思想观念。Objective:To analyze the clinical experience and academic thoughts of TCM master Li Jiren in the treatment of rheumar-thritis,rheumatoid arthritis,cervical spondylosis,waist Bi disease and gout and the clinical experience and academic thought of the principle of"treating different diseases with the same method"in the treatment of Bi disease.Methods:From January 2013 to De-cember 2018,Professor Li Jiren's prescriptions for the diagnosis and treatment of five diseases were collected,and the included da-ta were analyzed by Excel and network-based analysis of the compatibility of Chinese medicine prescriptions.Results:Through the analysis,the common Chinese medicine,the common couplet medicines and the compatibility network of Chinese medicine were obtained.Professor Li Jiren used rattan medicine,insect medicine,tonic medicine,blood activating medicine and spleen invigorat-ing medicine in the treatment of five diseases.Most of the medicines entered the liver,kidney and spleen channels.He paid atten-tion to the balance of cold and warm medicines.Qingluoyin prescription was commonly used in both rheumarthritis and rheumatoid arthritis,while Yishen Qingluo Huoxue Prescription is often used in rheumatoid arthritis.The medicines used for cervical spondylo-sis and waist Bi disease were very similar.Gout is different from other four diseases.Conclusion:By analyzing medication rules,it can be found that Professor Li's main treatment principles are to Guben Peiyuan,regulate cold and heat,regulate blood circulation to dredge channels,dispel wind and relieve pain,and treat different diseases with different emphases,which is embodied in the idea of"treating different diseases with the same method"within the scope of Bi disease.
